动态心电图在老年冠心病心肌缺血的意义

摘要

目的:探讨和研究动态心电图对老年冠心病心肌缺血患者临床诊断的特征及效果.方法:两组患者各60例行动态心电图检测,持续检测24h,分别记录SDNN、SDNN指数、RMSSD、和PNN50,并记录有症状组和无症状组患者缺血发作的时间、持续时间、心肌缺血阈变异性(IV)、每阵心肌缺血发作时最高心率(HMIT)和最低心率(LMIT).结果:研究组患者心率变异性指标SDNN、SDANN、RMSSD、PNN50无明显低于对照组,数据经统计学具有显著差异(P<0.05).有症状组患者心肌缺血发作时间、心肌缺血持续时间和心肌缺血阈变异性(IV)均明显低于有症状组,数据经统计学比较具有显著差异(P<0.05).结论:对于存在心肌缺血的患者,当阈值变化速度越快时,其心肌缺血的变异率随着特维变化情况加快研究成果.%Objective: To explore and study the dynamic ECG characteristics and effects of the clinical diagnosis of the elderly coronary heart disease in patients with myocardial ischemia. Method: Two groups with 60 cases were routine ambulatory electrocardiogram testing with 24-hour continuous detection. To record SDNN index, RMSSD, pNN50, time of the symptomatic group, asymptomatic patients with ischemic attack , duration, myocardialischemic threshold variability ( Ⅵ ), each array myocardial ischemic attack, maximum heart rate ( HMIT ) and the lowest heart rate ( LMIT ). Result: The heart rate variability index SDNN, SDANN, RMSSD, pNN50 of study group were significantly lower than control group, the data were statistically significant differences ( P<0. 05 ). Non-symptomatic patients with myocardial ischemic attack, myocardial ischemia, continuous time and myocardial Ⅵ were significantly lower than symptomatic group, the data were statistically significant differences ( P <0. 05 ). Conclusion: Dynamic ECG has great significance in the detection of myocardial ischemia in patients with coronary heart disease. It does not only protect patients' safety, but also provide adequate criteria for the clinical diagnosis and treatment.
