活血化瘀法联合西药治疗慢性盆腔炎的疗效及对炎性因子的影响

摘要

目的：分析探讨活血化瘀法联合西药治疗慢性盆腔炎的疗效及其对血清白介素－2（ IL－2），白介素－4（ IL－4），白介素－10（ IL－10）和肿瘤坏死因子α（ TNF－α）水平的影响。方法：回顾性分析我院自2013年1月至2014年1月收治的78例慢性盆腔炎患者的临床资料，按照随机数字表法分组，每组各39例，对照组患者单纯采用0．9％甲硝唑溶液静脉滴注，250mL1次／d，30d为1疗程；观察组患者在对照组的治疗基础上加用活血化瘀法治疗，小腹外敷2次／d，30min／次，30d为1疗程。观察两组患者治疗前后血清IL－2、IL－4、IL－10及TNF－α水平的变化，并进行临床疗效、症状改善情况及不良反应的评价。结果：治疗30d后，观察组临床总有效率92．31％明显高于对照组71．79％，（χ2＝5．571，P＜0．05）；治疗后两组患者白带增多、子宫压痛及盆腔积液等临床症状发生率明显下降，观察组下降程度优于对照组（χ2＝3．852，6．221，5．278，P＜0．05）；两组患者血清IL－2、IL－4、IL－10及TNF－α水平较治疗前均有明显改善，观察组改善程度明显优于对照组（ t＝3．771，2．564，2．103，4．428，P＜0．05）；对照组和观察组不良反应各发生3例（7．69％）和6例（15．38％），症状较轻，两组药物不良反应发生率比较差异无统计学意义（χ2＝1．130，P＞0．05）。结论：活血化瘀法联合西药治疗慢性盆腔炎具有较好的疗效及安全性，具有显著上调血清IL－2、IL－4、IL－10和降低TNF－α水平和抑制盆腔慢性炎症的作用。%Objective: To investigate the effect of promoting blood circulation combined with western medicine in the treatment of chronic pelvic inflammatory disease and its influence on serum inflammatory fac-tors, interleukin -2 ( IL-2) , interleukin -4, interleukin ( IL-4) -10 ( IL-10) and tumor necrosis factor alpha ( TNF-α) .Method:78 cases of chronic pelvic inflammatory disease cured in our hospital from Janu-ary 2013 to January 2014 were analyzed retrospectively, and divided into two groups randomly, 39 cases in each group, patients in the control group were treated with 0.9%metronidazole infusion, 250mL for once a day, 30 days as one course of treatment; observation group were treated with blood activating based on the control group, abdominal external application of twice a day, continued 30min, 30 days for one course of treatment.Changes of serum IL-2, IL-4, IL-10 and TNF-αlevels in the two groups before and after treat-ment, evaluation and clinical efficacy, and adverse reactions were observed.Result: After 30 days treat-ment, the clinical total effective rate of observation group was 92.31%,which was higher than that in control group 71.79%, (χ2=5.571, P<0.05);clinical symptoms ( leucorrhea, uterine tenderness and pelvic effu-sion) of patients in two groups were decreased significantly after treatment, the observation group was better than that of the control group (χ2=3.852, 6.221, 5.278 P<0.05) , serum IL-2, IL-4, IL-10 and TNF-αlevels were significantly improved compared with before treatment, the observation group improved signifi-cantly better than the control group ( t=3.771, 2.564, 2.103, 4.428, P<0.05);the control group and the observation group adverse reactions occurred in 3 cases (7.69%) and 6 cases (15.38%), the symptom was lighter, the adverse reaction of drugs of two groups rate had no significant difference (χ2=1.130, P>0.05) . Conclusion:Blood circulation promoting method combined with western medicine has good curative effect and safety, significantly increase serum IL-2, IL-4, IL-10;decrease TNF-αlevel and inhibition inflam-matory factor.
