微创开颅内窥镜辅助治疗老年高血压脑出血38例临床观察

Objective To observe the clinical effect of minimally invasive craniotomy endoscopic adjuvant therapy on senile hypertensive intracerebral hemorrhage. Methods 65 senile hypertensive cerebral hemorrhage patients were randomly divided into the treatment group and control group. 38 cases in treatment group received the keyhole endoscopic adjuvant therapy. 27 cases in control group received key hole evacuation of hematoma. The clinical effect, hematoma complete clearance rate and hematoma recurrence rate were observed and compared in two groups. Results Excellent rate of treatment group(86. 8% ) was higher than that in control group(74. 1% , P <0.05). Hematoma complete clearance rate and hematoma recurrence rate in treatment group(97.4% , 0% ) were superior to those in control group( 88. 9% , 7.4% , P < 0. 01). Conclusion Minimally invasive endoscopic assisted therapy has ideal effect on senile hypertensive cerebral hemorrhage, it is worthy of popularization and application.%目的 观察微创开颅内窥镜辅助治疗老年高血压脑出血的临床疗效.方法 将65例老年高血压脑出血患者随机分为2组.治疗组38例予神经内窥镜锁孔血肿清除术治疗,对照组27例予小骨窗血肿清除术治疗.观察2组临床疗效,比较血肿完全清除率、血肿复发率.结果 治疗组优良率86.8％,对照组优良率74.1％,2组优良率比较差异有统计学意义(P＜0.01),治疗组临床疗效优于对照组.治疗组血肿完全清除率97.4％,血肿复发率0；对照组血肿完全清除率88.9％,血肿复发率7.4％,治疗组血肿完全清除率、血肿复发率均低于对照组(P＜0.01).结论 微创开颅内窥镜辅助治疗老年高血压脑出血效果理想,值得临床推广应用.
