目的 分析不同民族喉癌的临床特点、手术治疗方式及其对预后的影响。方法 对1998-2009年于延边大学附属医院行喉癌手术的168例患者进行临床随访、病历资料分析。结果 朝鲜族中声门型喉癌的发病明显高于汉族，部分喉切除术患者拔管率为100％。全喉切除术4例发生咽瘘。随访149例，死亡37例，3年生存率为80.95％，5年生存率为70.24％。喉部分切除术患者的3年生存率为83.85％，5年生存率为74.62％，而喉全切除术患者的3年生存率为71.05％，5年生存率为52.63％。结论 2004-2009年入院喉癌患者较1998-2003年增加，朝鲜族和汉族喉癌的发生部位有差异；喉部分切除术是保留喉功能的根治性手术，喉癌患者经严格掌握适应证，正确选择术式，可提高术后生存率及生活质量。%Objective To analyze the clinical characteristics, surgical method and its impact on prognosis in patients with laryngeal carcinoma. Methods The clinical data of 168 patients with laryngeal carcinoma in the affiliated hospital of Yanbian university from 1998 to 2009 were retrospectively analyzed. Results Since 1998, the cases of laryngeal cancer has been increasing continually. The incidence in Korean ethnic minority group is significantly higher than that in Han ethnic group. The partial laryngectomy decannulation rate was 100%. During the follow-up (149 cases), 37cases died, with a three years survival rate of 80.95% and a five years survival rate of 70.24%. The three years survival rate and the five years survival rate of partial laryngectomy are 83.85% and 74.62%, and those of total laryngectomy are 71.05% and 52.63%, respectively. Conclusion The patients admitted with laryngeal carcinoma from 2004 to 2009 are increased compared to that from 1998 to 2003. The location of laryngeal carcinoma was different between Korean ethnic minority group and Han ethnic group. Partial laryngectomy is the preferred choice for preserving the laryngeal functions. The survival rate and life quality can be improved by proper selection of surgical indications.
