广州民警谷氨酰转肽酶与高血压和心电图缺血性ST-T改变关系的研究

         

摘要

Objective To explore the relationship among gamma glutamyl transpeptidase (GGT) and hyper-tension or ischemic ST-segment and (or) the T wave changes (ST-T changes) in electrocardiogram (ECG), and to study the effects of GGT on cardiovascular diseases in policemen in Guangzhou. Methods The blood pressure levels and ischemic ST-T changes in different levels of GGT groups divided by quartile of GGT of 2 120 Guangzhou policemen were analyzed. Results (1) The blood pressures rose with the GGT levels (β=0.006, P<0.05). There was a significant difference in the incidence between the hypertension group and non hypertension group (P<0.01). (2) The incidence of ischemic ST-T changes presented a similar V-shaped curve chart with GGT levels rising. The two high points of in-cidences of ischemic ST-T changes occurred in the low level group (<18 U/L) and the high level group (>39 U/L) of GGT;and the valley of incidence occurred in the medium relative low level group (18~26 U/L). There was a signifi-cant difference between incidence of ischemic ST-T changes and non ischemic ST-T changes (χ2= 13.031, P<0.01). Conclusion The incidence of hypertension rose with gamma glutamyl transpeptidase levels. Gamma glutamyl trans-peptidase is an independent risk factor of hypertension. Both low level and high level of Gamma glutamyl transpepti-dase may lead to an increase of incidences of ischemic ST-T changes in ECG.%目的:探讨谷氨酰转肽酶(GGT)与高血压及心电图缺血性ST段和(或)T波改变(ST-T改变)的关系,研究GGT对广州民警心血管事件的影响。方法对2120例广州民警四分位不同水平GGT的血压水平和心电图缺血性ST-T改变发生率进行分析。结果(1)受检者血压随GGT水平升高而升高。GGT水平与血压相关(β=0.006,P<0.05),高血压与非高血压组间比率差异有显著统计学意义(P<0.01)。(2)心电图缺血性ST-T改变发生率与GGT水平关系呈一近“V”形曲线图。发生率分别在GGT低水平组(<18 U/L)和高水平组(>39 U/L)出现高点,在中等较低水平组(18~26 U/L)出现波谷。心电图有、无缺血性ST-T改变的发生率之间差异有显著统计学意义(χ2=13.031,P<0.01)。结论高血压病发病率随谷氨酰转肽酶水平升高而升高,谷氨酰转肽酶为高血压病独立的危险因素。谷氨酰转肽酶在低水平及高水平可能导致心电图缺血性ST-T改变发生率升高。
