双层补片腹膜前修补治疗腹股沟滑疝的效果观察

摘要

Objective To investigate the clinical effect of preperitoneal hernioplasty with Shanyu bilayer mesh in the treatment of sliding inguinal hernia. Methods The clinical data of 48 patients of sliding inguinal hernia in our hospital from March 2010 to March 2013 were retrospectively analyzed. According to the operation mode, the patients were divided into traditional group (22 cases, using traditional preperitoneal hernioplasty for hernia repair), bi-layer mesh group (26 cases, applying preperitoneal hernioplasty with Shanyu bilayer mesh). The intraoperative and postoperative situations were compared between the two groups. Results The operation time, VAS score, rate of postoperative use of analgesics, postoperative ambulation time, postoperative recovery time were (23.7 ± 5.2) min, (0.86 ± 32), 11.54%, (16.8 ± 5.1) h, (35.5 ± 8.3) d in bilayer mesh group, respectively, significantly lower than those of traditional group (P<0.05). The two groups showed no statistically significant difference in the length of hospital stay, postoperative recurrence rate in one year (P>0.05). There were 9 cases (40.91%) of complications in traditional group and 2 cases (7.69%) of complications in bilayer mash group, with statistically significant difference between the two groups (χ2=7.443, P=0.006). Conclusion Reperitoneal hernioplasty with Shanyu bilayer mesh is advantageous over traditional hernioplasty in inguinal hernia repair, with shorter operation time, rapid postoperative recovery and fewer complications.%目的：探讨采用善愈双层补片腹膜前修补治疗腹股沟滑疝的临床效果。方法回顾性分析2010年3月至2013年3月我院结直肠肛门外科收治的48例腹股沟滑疝患者的临床资料，根据手术方式分为传统组(22例，采用传统疝修补手术)、双层补片组(26例，采用善愈腹膜前双层补片腹膜前修补手术)，比较两组患者的术中术后情况。结果双层补片组手术时间为（23.7±5.2）min、VAS评分为(0.86±32)分、术后使用镇痛剂率为11.54%、术后下床活动时间为(16.8±5.1) h、术后恢复工作时间为(35.5±8.3) d，均显著低于传统手术组(P<0.05)；两组患者的住院时间、术后1年复发率比较差异均无统计学意义(P>0.05)。传统组术后共出现并发症9例(40.91%)，显著高于双层补片组的2例(7.69%)(χ2=7.443，P=0.006)。结论善愈双层补片腹膜前修补治疗腹股沟滑疝较传统手术治疗具有手术时间短、恢复较快、术后并发症少等优点。
