异位性皮炎患者血清DHEA/DHEA-S水平测定及其临床意义

             

摘要

目的 测定异位性皮炎患者的血清去氢表雄酮(DHEA)和硫酸去氢表雄酮(DHEA-S)并评价其临床意义.方法 收集2015年8月至2016年3月期间我院皮肤科收治的异位性皮炎患者12例(观察组)与同期健康体检者20例(对照组),记录观察组异位性皮炎(AD)评分,检测和比较两组受检者的血清DHEA、DHEA-S及白介素-13(IL-13)、γ-干扰素(INF-γ)水平,并分析DHEA、DHEA-S与IL-13、INF-γ和AD评分的相关性.结果 观察组患者的血清DHEA平均为(1.01±0.23)ng/mL,低于对照组的(3.21±1.02)ng/mL,差异有统计学意义(P<0.05);观察组患者的DHEA-S平均为(45.08±12.32)μg/dL,低于对照组的(280.23±68.95)μg/dL,差异有统计学意义(P<0.05);观察组患者的IL-13为(40.23±5.34)pg/mL,明显高于对照组的(13.42±3.67)pg/mL,INF-γ为(40.29±6.64)pg/mL,明显低于对照组的(69.82±8.23)pg/mL,差异均具有统计学意义(P<0.05);DHEA/DHEA-S与AD评分呈显著正相关性(r=0.242、0.245,P<0.05),与IL-13呈显著负相关性(r=-0.532、-0.346,P<0.05),与INF-γ呈显著正相关性(r=0.512、0.249,P<0.05).结论 DHEA/DHEA-S水平降低可能是异位性皮炎的发病机制之一,检测DHEA/DHEA-S对异位性皮炎的诊断、病情评估及治疗具有指导意义.%Objective To measure serum dehydroepiandrosterone sulfate and serum dehydroepiandrosterone (DHEA) and DHEA-sulfate (DHEA-S) levels in patients with atopic dermatitis, and to evaluate its clinical significance. Methods A total of 12 patients with atopic dermatitis, who admitted to Department of Dermatology and Venereology of our hospital from August 2015 to March 2016, were selected as the observation group. At the same time, 20 healthy people were chosen as the control group. The scores of atopic dermatitis (AD) in the observation group were recorded. The levels of serum DHEA and DHEA-S, interleukin-13 (IL-13) and interferon-γ(IFN-γ) of the two groups were measured and compared, and the correlation between DHEA, DHEA-S and IL-13, INF-γ, AD score was analyzed. Results The aver-age serum DHEA level of the observation group was significantly lower than that of the control group, (1.01±0.23) ng/mL vs (3.21±1.02) ng/mL (P<0.05);the average DHEA-S level of the observation group was significantly lower than that of the control group, (45.08 ± 12.32) μg/dL vs (280.23 ± 68.95) μg/dL (P<0.05); the IL-13 level of the observation group was significantly higher than that of the control group, (40.23±5.34) pg/mL vs (13.42±3.67) pg/mL (P<0.05);the INF-γ level of the observation group was significantly lower than that of the control group, (40.29 ± 6.64) pg/mL vs (69.82±8.23) pg/mL (P<0.05). DHEA/DHEA-S had the significant positive correlation with AD (r=0.242, 0.245, P<0.05) and INF-γ(r=0.512 and 0.249, P<0.05), and had the significant negative correlation with IL-13 (r=-0.532,-0.346, P<0.05). Conclusion The decrease of DHEA/DHEA-S level may be one of the pathogenesis of atopic dermatitis. The measurement of DHEA/DHEA-S has guiding significance in the diagnosis, evaluation and treatment of atopic dermatitis.
