目的：探讨纯磨玻璃密度(pGGO)浸润性肺腺癌的CT征象。方法回顾性分析石家庄市第一医院2013年12月至2016年1月期间收治的66例(66个病灶)经手术病理证实的pGGO浸润性肺腺癌的CT征象，包括病灶形态、边缘、内部及邻近结构改变等。结果66个病灶中形态不规则45例，瘤-肺界面清晰62例，毛刺征34例，分叶征13例，空泡征47例，空气支气管征43例，血管集束征31例，胸膜凹陷征27例。结论 CT检查发现pGGO时，若病灶瘤-肺界面清晰、形态不规则、可见毛刺征、空泡征、空气支气管征时要高度怀疑浸润性肺腺癌的可能。%Objective To investigate the CT manifestations of invasive adenocarcinoma presented as pure ground glass opacity (pGGO). Methods The CT manifestations of 66 cases (66 lesions) presented as pGGO, including the lesion's shape, margin, internal and adjacent structure changes, were analyzed retrospectively, which enrolled from December 2013 to January 2016 in the First Hospital of Shijiazhuang. All the lesions were confirmed by surgery and pa-thology. Results Among the 66 lesions, there were 45 cases with irregular shape, 62 cases with clear tumor-lung inter-face, 34 cases with speculation, 13 cases with lobulation, 47 cases with vacuole sign, 43 cases with air bronchogram, 31 cases with vascular convergence and 27 cases with pleural indentation. Conclusion pGGO lesions with clear tu-mor-lung interface, irregular lesion's shape, speculation, vacuole sign and air bronchogram, under CT scan should be highly suspected for invasive pulmonary adenocarcinoma.
