新活素对心肌梗死合并心力衰竭患者血管内皮功能的影响

             

摘要

目的 观察新活素对心肌梗死合并心力衰竭患者血管内皮功能的影响.方法 收集2012年6月至2015年5月我院心内科救治的心肌梗死合并心力衰竭患者180例,按随机数字表法将其分观察组与对照组,两组均90例.对照组患者给予常规药物治疗,观察组患者在常规药物治疗的基础上静脉注射新活素治疗.分别于治疗前和治疗2周后观察两组患者肱动脉内皮依赖性的舒张功能、血清中一氧化氮(NO)及内皮素1的含量.结果 治疗2周后观察组患者的肱动脉内皮依赖性舒张功能为(15.24±2.35),明显高于与对照组的(8.98±1.28),差异有统计学意义(P≤0.05);治疗2周后观察组患者血清中NO的含量为(48.89±5.28)μmol/L,明显高于对照组的(34.81±3.51)μmol/L,而内皮素1的含量为(69.85±4.45) ng/L,明显低于对照组的(83.84±6.86) ng/L,差异均有统计学意义(P<0.05).结论 新活素通过增加心肌梗死合并心力衰竭患者血清中NO的含量,降低内皮素1的含量,能明显改善心肌梗死合并心力衰竭患者血管内皮功能.%Objective To observe the effect of natriuretic peptide on vascular endothelial function of patients with heart failure complicating myocardial infarction. Methods One hundred and eighty patients with heart failure complicating myocardial infarction were collected from June 2012 to May 2015 in Department of Cardiology in our hos-pital, which were divided into experimental group (n=90) and control group (n=90) according to random number table. The control group was given conventional drug treatment, and the experimental group was given natriuretic peptide on the basis of conventional drug treatment. The brachial artery endothelial dependent diastolic function, the content of NO and eddthelin 1 in the two group were observed before and after 2 weeks of treatment. Results Two weeks after treatment, the brachial artery endothelial dependent diastolic function, the content of NO in the experimental group [(15.24 ± 2.35), (48.89 ± 5.28)μmol/L] were significantly higher than those in the control group [(8.98 ± 1.28), (34.81 ± 3.51)μmol/L], and the content of eddthelin 1 in the experimental group [(69.85±4.45) ng/L] was significantly lower than that in the control group [(83.84 ± 6.86) ng/L]. The differences were all statistically significant (P<0.05). Conclusion Natriuretic peptide can significantly improve endothelial function of the patients with heart failure complicating myocardial infarction by in-creasing the content of NO and reducing the content of endothelin 1in serum.
