米非司酮对子宫肌瘤组织中表皮生长因子及Cx43表达的影响

             

摘要

目的 探讨米非司酮对子宫肌瘤组织中表皮生长因子(EGF)及上调间隙连接蛋白Cx43表达的影响.方法 从海口市中医院妇产科2015年1~12月收治的子宫肌瘤患者中选取符合入组条件的子宫肌瘤患者92例,按随机数表法分为对照组和观察组,每组46例.另选取同期在我院因卵巢肿瘤行手术治疗的40例正常子宫肌组织作为正常组.观察组于术前5 d开始予以米非司酮25 mg,2次/d,口服,连续服用5 d,停药后立即行手术治疗.正常组与对照组在确诊后立即行全子宫切除术.治疗结束后,比较三组受检者子宫肌瘤组织及正常子宫肌组织中EGF及Cx43的表达情况.结果 ①对照组在增生期子宫肌瘤组织的EGF mRNA为(0.31±0.27),与正常组增生期的(0.27±0.14)比较差异无统计学意义(P>0.05);②对照组在分泌期子宫肌瘤组织的EGF mRNA为(1.45±0.43),与正常组分泌期的(0.53±0.25)比较差异有统计学意义(P<0.05);③观察组在增生期子宫肌瘤组织的EGF mRNA为(0.26±0.11),与对照组增生期的(0.31±0.27)、正常组增生期的(0.27±0.14)比较差异无统计学意义(P>0.05);而观察组在分泌期子宫肌瘤组织中EGF mRNA为(0.54±0.23),与对照组分泌期的(1.45±0.43)比较,差异有统计学意义(P<0.05);④对照组增生期子宫肌瘤内Cx43蛋白表达与分泌期比较,差异具有统计学意义(P<0.05);正常组患者增生期子宫肌内Cx43蛋白表达与分泌期比较,差异无统计学意义(P>0.05);⑤观察组增生期子宫肌瘤Cx43蛋白表达与对照组比较,差异无统计学意义(P>0.05);而分泌期观察组子宫肌瘤内Cx43蛋白表达与对照组比较,差异有统计学意义(P<0.05).结论 米非司酮能够明显抑制子宫肌瘤的生长,对临床诊治具有指导意义.%Objective To investigate the effect of mifepristone on epidermal growth factor (EGF) and up-regu-lation of connexin43 (Cx43) in uterine fibroids. Methods Ninety-two patients with uterine fibroids in Department of Obstetrics and Gynecology in our hospital from January 2015 to December 2015 were randomly divided into control group and observation group, with 46 cases in each group. Forty patients undergoing surgical treatment for ovarian tu-mor were selected as the normal group. The observation group received mifepristone 25 mg for 5 consecutive days be-fore surgery, two times per day. The control group and the normal group received hysterectomy immediately after con-firmed diagnosis. After treatment, the expression of EGF and Cx43 in uterine fibroids and normal uterine muscle were compared between the three groups. Results ①There was no significant difference in the level of EGF mRNA in the proliferative phase between the control group and the normal group, (0.31 ± 0.27) vs (0.27 ± 0.14), P>0.05.②The EGF mRNA level in progestational stage was (1.45±0.43) in the control group, versus (0.53±0.25) in normal group (P<0.05).③The level of EGF mRNA in proliferative stage in the observation group was (0.26 ± 0.11), versus (0.31 ± 0.27) in the control group and (0.27±0.14) in normal group (P>0.05);the EGF mRNA level in progestational stage was (0.54±0.23) in the observation group, significantly lower than (1.45 ± 0.43) in the control group (P<0.05).④The difference in Cx43 between in proliferative stage and in progestational stage was significant in the control group (P<0.05), but not signifi-cant in normal group (P>0.05).⑤The level of Cx43 protein in proliferative stage in the observation group showed no significant difference with that in the control group (P>0.05), but the level in progestational stage showed statistically significant difference between the two groups (P<0.05). Conclusion Mifepristone can significantly inhibit the growth of uterine fibroids.
