尿石清合剂对下焦湿热型尿路结石患者上尿路动力学的影响

摘要

Objective To discuss the clinical effects of Niao-Shi-Qing Mixture on upper urinary tract dynam-ics in patients with lithangiuria. Methods From February 2015 to January 2016, 150 patients with lithangiuria from Zhongshan Hospital of Traditional Chinese Medicine were divided into the observation group (n=50), the control group (n=50), and the blank group (n=50) by random number table, which were treated with Niao-Shi-Qing Mixture, Niao-Shi-Tong Pill, no drugs after super-micro-percutaneous nephrolithotomy (SMP). Then postoperative renal pelvic pressure, and the changes in urinary volume, serum ions (Mg2+, Ca2+, P), serum creatinine (SCr), blood urea nitrogen (BUN), oxalic acid and citric acid before and after treatment were compared between the three groups. Results The postoperative renal pelvic pressure in the observation group was (12.8±2.4) mmHg, which was significantly higher than (9.0±2.5) mmHg in the control group and (6.2±2.3) mmHg in the blank group (P<0.05). Urinary volume after the treat-ment in the observation group was (2645 ± 725) mL, which was significantly more than (2435 ± 755) mL in the control group and (1935±755) mL in the blank group (P<0.05). Mg2+, SCr, BUN, and citric acid levels after the treatment in the three groups were significantly increased compared with before treatment (P<0.05), and the levels in the observation group were significantly higher than those in other two groups (P<0.05). Ca2+, P, and oxalic acid after the treatment in the three groups were significantly reduced (P<0.05), and the levels in the observation group were significantly lower than those in other two groups (P<0.05). Conclusion Application of Niao-Shi-Qing Mixture after SMP can significantly im-prove the renal pelvic pressure, increase urinary volume, and ameliorate serum ion levels.%目的 探讨尿石清合剂对下焦湿热型尿路结石患者上尿路动力学的影响.方法 选择中山市中医院2015年2月至2016年1月收治的150例尿路结石患者,以随机数字表法分为观察组、对照组和空白组,每组50例,所有患者均给予超细经皮肾镜取石术(SMP)治疗,术后观察组给予尿石清合剂,对照组给予尿石通丸,空白组术后不予排石药物治疗,比较三组患者术后肾盂压力,治疗前后的尿量变化,以及血清离子(Mg2+、Ca2+、P)、血肌酐(SCr)、血尿素氮(BUN)、草酸和枸橼酸的变化.结果 观察组患者治疗后的肾盂压力为(12.8±2.4)mmHg,明显高于对照组的(9.0±2.5)mmHg和空白组的(6.2±2.3)mmHg,差异有统计学意义(P<0.05);观察组患者治疗后的尿量为(2645±725)mL,明显高于对照组的(2435±755)mL和空白组的(1935±755)mL,差异有统计学意义(P<0.05);治疗后,三组患者的Mg2+、SCr、BUN、枸橼酸显著升高,且观察组明显高于对照组和空白组,三组患者的Ca2+、P、草酸显著降低,且观察组明显低于对照组和空白组,差异均有统计学意义(P<0.05).结论 SMP术后加服尿石清合剂可显著提高肾盂压力,增加尿量,改善血清各离子水平,疗效显著.
