激光针灸联合雷诺考特治疗儿童阻塞性呼吸睡眠暂停低通气综合征疗效观察

摘要

目的 观察激光针灸联合雷诺考特治疗儿童阻塞性呼吸睡眠暂停低通气综合征(OSAHS)的疗效.方法 选取惠州市第二妇幼保健院儿科2014年1月至2016年1月收治的64例OSAHS患儿,采用随机数字表法将患儿分为对照组和观察组,各32例.对照组给予夜间低流量给氧及雷诺考特治疗,观察组在对照组治疗的基础上给予激光针灸治疗,连续治疗4周.比较两组患儿的临床治疗效果、治疗前后的呼吸暂停低通气指数(AHI)、夜间血氧饱和度(SpO2)、血清白细胞介素-6(IL-6)与肿瘤坏死因子-α(TNF-α)的含量水平.结果 观察组患儿的治疗总有效率为93.75%,明显高于对照组的75.00%,差异有统计学意义(P<0.05);观察组患儿治疗后AHI明显低于对照组[(6.23±3.07)次/h vs(8.14±4.29)次/h],SpO2明显高于对照组[(91.42±5.86)%vs 87.34±6.21)%],差异均有统计学意义(P<0.05);观察组和对照组患儿治疗后的IL-6[(17.82±6.37)pg/mL vs(28.56±4.61)pg/mL]与TNF-α[(7.56±1.47)pg/mL vs(12.50±2.38)pg/mL]水平比较,观察组明显低于对照组,差异均有统计学意义(P<0.05).结论 激光针灸联合雷诺考特能够明显改善OSAHS患儿的临床症状,其不仅能够减少患儿睡眠呼吸暂停暂停次数,还能通过改善患者炎症反应提高疗效.%Objective To observe the curative effect of laser acupuncture and rhinocort in treating the obstruc-tive sleep apnea-hypopnea syndrome (OSAHS) in children. Methods Sixty-four patients with OSAHS from the Depart-ment of Pediatrics in the Huizhou Municipal No.2 Maternal and Children Hospital from Januanry 2014 to Januanry 2016 were divided into the observation group and control group by random number table, with 32 patients in each group. The continuous administration of low flow oxygen at night and the rhinocort was given to the patients in control group;based on the the therapy in the control group, additional laser acupuncture were given to those in observation group, and all pa-tients were continuously treated for 4 weeks. The clinical signs and symptoms including the apnea hypopnea index (AHI), the blood oxygen saturation (SpO2) and the serum concentrations of interleukin (IL)-6 and tumor necrosis factor (TNF)-α were compared. Results The total effective rate in observation group was 93.75%, which was higher than 75.00%in the control group (P<0.05). AHI in the observation group was obviously less than in the control group (6.23± 3.07) time/h vs (8.14 ± 4.29) time/h, while SpO2 in the observation group was significantly higher than in the control group, (91.42 ± 5.86)% vs (87.34 ± 6.21)%, (P<0.05). Moreover, IL-6 (17.82 ± 6.37) pg/mL vs (28.56 ± 4.61) pg/mL and TNF-α(7.56±1.47) pg/mL vs 12.50±2.38 pg/mL in the observation group were remarkably less than those in the control one (P<0.05). Conclusion The combination of laser acupuncture and rhinocort can significantly improve the clinical symptoms of the children with OSAHS, which can not only decrease significantly frequency of central sleep apnea but also improve the inflammatory response and improve the curative effect.
