丹红注射液对脑梗死患者脑血流动力学和血液流变学的影响

         

摘要

目的 探讨丹红注射液对急性脑梗死患者脑血流动力学和血液流变学的影响.方法 选择我院神经内科于2015年10月至2016年7月期间收治的62例急性脑梗死患者为研究对象,根据随机数字表法将患者分为两组,每组31例,对照组患者予抗血小板等常规治疗,观察组在对照组治疗的基础上联合应用丹红注射液治疗,疗程14 d.比较两组患者治疗前后的脑血流动力学和血液流变学指标的变化.结果 治疗后,两组患者大脑后动脉收缩期最大血流速度(Vs)、平均血流速度(Vm)均较治疗前显著增加,阻力指数(RI)显著下降,且观察组明显优于对照组,差异均有统计学意义(P<0.05);治疗后两组患者的全血比黏度(高切、中切、低切)、血浆黏度及纤维蛋白原均较治疗前明显降低,且观察组较对照组降低更明显,差异均有统计学意义(P<0.05).结论 丹红注射液治疗急性脑梗死可以明显降低患者的血液黏稠度,增加大脑后动脉等颅内血管的血供,对患者预后的改善有利.%Objective To investigate the effect of Dan Hong Injection on cerebral hemodynamics and hemorhe-ology in patients with acute cerebral infarction. Methods From October 2015 to July 2016, 62 patients of acute cerebral infarction in Department of Neurology of our hospital were studied. According to the random number table method, they were divided into two groups, each with 31 cases. The control group was treated with antiplatelet and conventional thera-py, and the observation group was treated additionally by Dan Hong injection, for 14 days. The changes of cerebral hemo-dynamics and blood rheology indexes were compared between the two groups before and after treatment. Results After treatment, the systolic peak flow velocity (Vs) and mean blood flow velocity (Vm) of posterior cerebral artery were sig-nificantly increased in both groups, and the resistance index (RI) decreased significantly, compared with those before treatment. The above indexes in the observation group were significantly better than those in the control group (P<0.05). After treatment, the blood rheology indexes including blood viscosity (high, middle and low cut), plasma viscosity and fi-brinogen in the two groups were significantly lower than before treatment, and the levels in the observation group were significantly lower than those in the control group (P<0.05). Conclusion Dan Hong injection in the treatment of acute cerebral infarction can significantly reduce blood viscosity, increase the blood supply of posterior cerebral artery, and im-prove the prognosis.
