胃癌组织中EB病毒、ARID1A及PIK3CA的表达及意义

             

摘要

目的 探讨胃腺癌组织中EB病毒(EBV)、AT丰富结合域1A基因(ARID1A)和磷脂酰肌醇-3-激酶催化亚基(PIK3CA)的表达及意义.方法 筛选2016年1月至2017年11月在中南大学湘雅医学院附属海口医院经手术切除的胃癌组织蜡块86例作为胃癌组,同时选取癌旁胃组织43例作为对照组,运用EBER原位杂交法行EBV检测,免疫组化SP法行ARID1A和PIK3CA的检测,并将胃癌组按临床病理特征进行分组,统计分析EBV、ARID1A和PIK3CA三者在各组间表达的差异及相关性.结果 胃癌组中EBV表达阳性例数为10例(11.63%),对照组中EBV表达全为阴性,差异有统计学意义(P<0.05);胃癌组中8例有ARID1A表达缺失(9.30%),而对照组中为0例,差异具有统计学意义(P<0.05);PIK3CA在胃癌组中有67例表达明显增强(77.90%),而对照组中仅有3例为强表达(27.91%),两组间比较差异有统计学意义(P<0.05);结合患者的临床特征进行分析,发现ARID1A与PIK3CA的表达均与肿瘤的浸润深度有关(P<0.05),而与年龄、性别、分化程度、组织学分型及淋巴结转移无关(P>0.05);相关性分析结果表明,在EBV阳性胃癌(EBVaGC)组织和EBV阴性胃癌(EBVnGC)组织中ARID1A与PIK3CA两者之间的表达均呈负相关(r=-0.629,r=-0.419,P<0.05).结论 部分胃癌的发生与EBV感染、ARID1A和PIK3CA基因突变相关,为探索防癌治癌的新思路和新方法提供理论依据.%Objective To investigate the expression of Epstein-Barr virus (EBV), AT-rich interaction domain 1A (ARID1A) and phosphatidyli-nositol 3-kinase catalytic alpha (PIK3CA) in gastric adenocarcinoma, and toandyze their significance. Methods A total of 86 cases of primary gastric cancer tissues obtained from patients, who underwent re-section in Haikou Hospital Affiliated to Xiangya School of Medicine of Central South University during Jan. 2016 and Nov. 2017, were selected as the experimental group, and 43 matched adjacent non-tumor tissues were involved in our study as the control group. EBV infection was examined by in situ hybridization, and the expression of ARID1A and PIK3CA were checked by using immunohistochemistry SP method. The gastric cancer tissues were grouped by clinicopathological features, the differences and relations of EBV, ARID1A and PIK3CA in each group were analyzed. Results There were 10 cases of EBV infection among 86 gastric cancer cases (11.63%), while none case was found EBV infection among the control group (P<0.05). There were 8 cases of gastric cancer lacking the expression of ARID1A (9.3%), while no case lacked the expression of ARID1A in control group (P<0.05). Nevertheless, the expression of PIK3CA was strongly increased in 67 cases (77.9%) of gastric cancers, and only 3 cases of control group had strong expression of PIK3CA (27.91%), P<0.05. The clinical features of 86 cases of gastric cancer were retrospectively analyzed, which showed that the expression of ARID1A and PIK3CA were significantly associated with infiltration, but showed no significant difference in age, sexuality, differentiation, histological classification and lymphatic metastasis (P>0.05). The results of correlation analysis of ARID1A and PIK3CA revealed that these two genes were negatively related among EBV positive gastric cancers and EBV negative gastric cancers (r=-0.629, r=-0.419, P<0.05). Conclusion A few gastric cancer cases are related to EBV infection and the mutation of ARID1A and PIK3CA, which may provide clue for the prevention and treatment of gastric cancer.
