连续床边脑电图监测对昏迷患儿预后的评估价值

     

摘要

Objectives To explore the prognostic value of Continous dynamic electroencephalogram (EEG) in comatose neonates.Methods Continous EEG were completed by brain function monitor with 60 cases of comatose neonates.Clinical outcome was evaluated using the modified Glasgow Outcome Scale, to analyze the relationships between CEEG/Glasgow grading and prognosis of neonates.Results 28 neonates (46.7%) with grade II EEG were cured with good prognoses.In all 10 neonates(16.7%) which were classified with grade III EEG, 2 cases werecured, 8cases were poor outcome, but without death. 2 cases were poor prognosis, and 15 died among the 17 neonates (28.3%) with grade IV EEG. However, 5 neonates (8.3%) with grade II EEG were died. Spearman correlation coefficients between electroencephalogram(EEG)electroencephalogram (EEG) and prognosis was 0.953,P<0.001. The higher the EEG classification,the lower the Glasgow score,and Spearman correlation coefficients between them was 0.84,P<0.001. Conclusion The detection of continuous EEG can reflect the state of brain function, and EEG might be of certain value in the evaluation of prognosis in comatose neonates.%目的:探讨连续脑电图监测对昏迷患儿预后的评估价值。方法采用脑功能监护仪对60例昏迷患儿进行床边脑电图监测(CEEG), Glasgow评分评估昏迷状况,分析CEEG/Glasgow评分与预后的相关性。结果28例(46.7%)II级脑电图患儿全部预后良好转归;10例(16.7%)III级患儿预后良好转归2例,不良转归8例,无死亡;17例脑电图IV级患儿(28.3%)预后不良转归2例,死亡15例;5例(8.3%)脑电图V级患儿死亡5例。脑电图与预后的Spearman等级相关系数为0.953,P<0.001;脑电图分级越高,Glasgow评分越低,并且Glasgow评分与预后相关,Spearman系数为0.84,P<0.001。结论连续床边脑电图检测可实时反映脑功能的状态,对昏迷患儿预后的评估有指导价值。
