60例产妇精神因素对分娩方式及其产后出血影响的临床观察

摘要

Objective To investigate the effect of maternal psychological factors on the mode of delivery and postpartum hemorrhage, to guide the clinical treatment. Methods From 2012 May -2013 year in May included, maternal data of 60 cases in our hospital clinic observation, on maternal mental state, mode of delivery, postpartum hemorrhage were retrospectively analyzed. Hamilton Anxiety Scale(HAS), depression scale(HDS), carried on the evaluation to the maternal mental state. The mode of delivery were divided into group cesarean section, labor group, with 30 patients in each group; according to the postpartum hemorrhage were divided into postpartum hemorrhage, postpartum hemorrhage group and non group 2 groups. In strict accordance with the HAS, HDS scale, evaluation of maternal anxiety, depression and other mental state, the prenatal mental state level, to the mode of delivery and the postpartum hemorrhage assessment ready. Results The anxiety and depression of the cesarean section maternal score, signiifcantly higher than natural delivery group(P<0.01);postpartum hemorrhage group anxiety, depression score, also signiifcantly higher than natural delivery group(P<0.01); fully shows the possibility of maternal psychological factors inlfuence on the mode of delivery and postpartum hemorrhage. Conclusion Mental factors on clinical mode of delivery and postpartum hemorrhage has certain inlfuence, should advance to maternal spirit.%目的：探讨产妇精神因素对分娩方式及产后出血的影响，指导临床治疗工作。方法共收录2012年5月至2013年5月，60例在我院产科接受临床观察的产妇资料，对产妇精神状态、分娩方式、产后出血等情况回顾性分析。采用汉密顿焦虑量表（HAS）、抑郁量表（HDS）等方式对产妇精神状态进行测评。本次分娩方式平均分为剖宫产组、顺产组等，每组30例；按照产后出血情况分为产后出血组、产后无出血组等2组。严格按照HAS、HDS等量表，对产妇焦虑、抑郁等精神状态进行测评，掌握其产前精神状态水平，为分娩方式及产后出血评估做好充分的准备。结果本次剖宫产组产妇的焦虑及抑郁评分，明显高于顺产组（P＜0.01）；产后出血组焦虑、抑郁等评分，同样明显高于顺产组（P＜0.01）；充分说明产妇精神因素对分娩方式及产后出血影响的可能性。结论精神因素对临床分娩方式及产后出血存在一定影响，需提前做好产妇精神疏导工作。
