IOL-Master测量高度近视白内障人工晶状体度数的准确性研究

     

摘要

目的:比较非接触式光学相干生物测量仪(intraocularlen-master,IOL-Master)与传统超声生物测量方法在测量高度近视白内障眼人工晶状体度数的准确性,评价IOL-Master的特性及其临床应用价值。方法对48例62只高度近视眼并发白内障患者施行超声乳化白内障吸出术。术前分别用IOL-Master、传统超声生物测量仪测量眼轴长度,用IOL-Master测量角膜曲率,根据测量结果使用SRK/T公式计算人工晶状体度数,术中植入可折叠人工晶状体。术后1个月随诊检查视力及眼屈光度。结果IOL-Master和传统超声生物测量法,测得的眼轴长度分别为(29.32±1.48)mm和(28.78±1.54)mm,二者对比差异有显著性(P<0.05);术后1个月的平均绝对屈光误差两组对比差异有显著性(P<0.05)。结论IOL-Master是一种高精确性、安全可靠、操作简单的人工晶状体度数测量工具。尤其对高度近视白内障手术人工晶状体的选择有重要临床价值。%Objective To compare the measurement of axial length(AL)by IOL-Master and contact ultrasonic(US) axial scan(A-scan)in high myopia with cataract. Methods 48 patients(62 eyes)of high myopia with cataract were comprised in this study. Before the phacoemulsiifcation,the axial length and corneal curvature were measured with IOL-Master and contact A-scan and respectively. Keratometric power was measured by IOL-Master.IOL power calculation was carried out according to SRK/T formula in the basis of IOL-Master and contact A-scan and respectively. Visual acuity and ocular diopter were examined during the followed up time for one month. Results The axial length was(28.78±1.54)mm by contact A-scan,(29.32±1.48)mm IOL-Master respectively. There were signiifcant difference between contact A-scan and IOL-Master(P<0.05).Mean absolute refractive error (MAFE) after one month was (0.431±0. 168)D(0~1.00)D by IOL-Master, and(0.682±0.27)D(0~1.15)D by contact A-scan. There was a signiifcant difference between contact A-scan and IOL-Master(P<0.05). Conclusions The IOL-Master is a non-contact, accurate, safe tool for calculating IOL power, and it is more accurate than contact A-scan,especially for high myopia.
