糖尿病肾损害的评价指标

摘要

目的：探讨联合检测血清胱抑素 C（CysC）及尿微量白蛋白/肌酐（A/C）在评估糖尿病早期肾损害中的诊断价值。方法选取2008年6月至2011年12月间包头一附院收治的220例2型糖尿病（DM2）患者，根据尿白蛋白排泄率（UAER）分为三组：DM 组， DN1组、DN2组，同时选择同期我院健康体检者75例作为正常对照组（NC 组），同时检测血 CysC 及尿 A/C，对检测结果行统计学分析。结果与 NC 组比较，其余实验组血 CysC 及尿 A/C 均升高，有统计学差异（P ＜0.05），且 DN2组血 CysC 及尿 A/C 明显高于 DN1组（P＜0.05），各组血 CysC 及尿 A/C 联合检测时阳性率升高，且两项指标间存在显著正相关；结论除目前公认的评价糖尿病早期肾损害的指标尿 A/C 外，血 CysC 也可作为评价糖尿病早期肾损害的敏感指标，且联合检测具有更高的临床价值。%Objective To study the diagnostic value of serum cystatin C(CysC) and microalbuminuria/creatinine(A/C) combined detection in the diagnosis of early renal damage with diabetes patients. Methods 220 cases patients with diabetes mellitus who visited First affiliated hospital of Baotou medical college (between 2008-06 and 2011-12) were enrolled, They were divided into DM group, DN1 group and DN2 group according to urinary albumin excretion rate(UAER). And 75 cases of healthy people were used as normal group(NC group). CysC and urine A/C were detected. Results The serum CysC and urine A/C of other groups were higher compared with NC group(P<0.05), and group DN2 were higher than group DN1(P<0.05).The combined detection of each index can improved the positive rate, and two indexes had significantly positive correlations. Conclusion In addition to the index of early diabetic renal damage urine A/C which was currently accepted. The serum CysC also can be used as sensitive indicators of early renal damage of diabetes, and combined detection has higher clinical value.
