内镜第三脑室底造瘘术的体会

摘要

目的：探讨内镜第三脑室底造瘘术治疗梗阻性脑积水的体会和疗效。方法回顾分析2014年1月至2015年6月我院内镜第三脑室底造瘘术治疗的18例梗阻性脑积水。结果18例梗阻性脑积水中，完成内镜第三脑室底造瘘术16例，中转脑室-腹腔分流术2例，随访4~12个月，症状明显缓解13例（有效率72.2%）、症状无变化2例、症状复发2例、症状加重添加脑室-腹腔分流术1例；复查头颅CT、MRI，脑室缩小7例（38.9%）、无明显变化9例、略变大2例。结论内镜第三脑室底造瘘治疗梗阻性脑积水微创、有效、并发症少。%Objective To explore the experience and effect of endoscopic third cerebral ventricle fistula in the treatment of obstructive hydrocephalus. Methods A retrospective review of 18 cases of obstructive hydrocephalus in our hospital from January 2014 to June 2015. Results 4 to 12 months of follow-up, the symptoms were relieved in 13 cases (effective rate is 72.2%), no change in symptoms in 2 cases, recurrence of symptoms in 2 cases, the symptom is aggravating add ventriculo peritoneal shunt in 1 case;CT and MRI decrease in size of the ventricle in 7 cases (38.9%), no obvious change in 9 cases, slightly bigger in 2 cases. Conclusion Endoscopic third ventricle floor of the treatment of obstructive hydrocephalus, minimally invasive, effective, less complications.
