广西巴马小型猪浅低温心脏不停跳体外循环模型的麻醉管理

         

摘要

Objective To investigate the effects of anesthetic management of the establishment of mild hypothermic cardiopulmonary bypass( CPB ) on beating heart for Guangxi Bama miniature pig. Methods Thirteen Bama miniature pigs were chosen, 10 pigs were used for CPB, another 3 pigs were used for sampling blood. Atropine, ketamine and midazolam were intramuscular injected before tracheal intubation through dicaine surface anesthesia. Intravenous injection with propofol, fentanyl and vecuronium and inhaled isoflurane intermittent were continued for anesthesic maintain. The effects of basic anesthesia and general maintain were evaluated by grade appraisal. The nasopharyngeal temperature was kept at 32℃ to 34℃ and heart rate should be maintained 50 bpm to 80 bpm during CPB,sinus rhythm and empty beating heart. Results The model of mild hypothermic CPB on beating heart was established successfully in nine pigs, one pig was died of hypoxia induced by intubation to esophagus. Basic anesthesia of grade Ⅰ for seven( 70.0% ) pigs,of grade Ⅱ for one( 10% ) pig,and of grade Ⅲ for one( 10% ) pig. General maintain of gradeⅠfor eight( 80.0% ) pigs,and of grade Ⅱ for one( 10% ) pig. Conclusion It is important to choose suitable anesthetic and evaluate methods to establish mild hypothermic CPB on beating heart for Guangxi Bama miniature pig.%目的 探讨建立广西巴马小型猪浅低温心脏不停跳体外循环模型的麻醉管理方法.方法 广西巴马小型猪13只,10只用于实验,3只用于取血预充.基础麻醉采用阿托品、氯胺酮和咪唑安定肌肉注射,丁卡因喷喉表面麻醉后气管插管,麻醉维持采用丙泊酚、芬太尼、维库溴胺持续静注和间断吸入异氟醚.采用分级评估方法 对基础麻醉、全麻维持的效果进行评估.鼻咽温32℃~34℃体外循环,维持心率50~80次/min,窦性节律空跳.结果 成功建立浅低温心脏不停跳体外循环模型9只,失败1只(在气管插管时误插入食道缺氧死亡).基础麻醉Ⅰ级7只(70.0%),Ⅱ级1只(10.0%),Ⅲ级1只(10.0%).全麻维持Ⅰ级8只(80.0%),Ⅱ级1只(10.0%).结论 选择合适的麻醉方法 和效果评估方法,是广西巴马小型猪浅低温心脏不停跳体外循环模型建立的关键.
