甲状腺转录因子-1在肺癌组织中的表达及临床意义

摘要

Objective To investigate the expression and diagnostic significance of thyroid transcription factor 1( TTF-1) in lung cancer tissues .Methods The expression of TTF-1 was detected by immunohistochemical staining in 50 cases of primary lung cancer ( including 23 cases of adenocarcinoma ,14 cases of squamous carcinoma ,9 cases of small-cell lung cancer ,and 4 cases of large-cell lung cancer ) and para-carcinoma tissues as well as 21 cases of metastatic lung adenocarcinomas from digestive tract and ovaries .The diagnostic and differential diagnostic values of TTF-1 in lung cancer were assessed .Results The positive expression rates of TTF-1 were 73 .9%( 17/23 ) in adenocarcinoma ,0 in squamouse cell carcinoma,77.8%(7/9)in small-cell lung cancer tissues and 50.0%(2/4)in large-cell lung cancer tissues,0 in metastatic lung adenocarcinomas ,respectively.The positive expression rates of TTF-1 in adenocarcinoma and small-cell lung cancer tissues were higher than that in squamouse cell carcinoma tissues (P<0.05).In para-carcinoma tissues of primary lung cancer ,normal or proliferous alveolar epithelium were less staining and had fewer positive cells (<20%) .The expressions of TTF-1 were all negative in the para-carcinoma tissues of secondary lung cancer .The positive expression rate of TTF-1 in lung cancer tissues wasn′t related with patients′age,gender,tumor size,tumor location(P>0.05).Conclusion TTF-1 can be used as a specific indicator for the differential diagnosis of primary or secondary lung adenocarcinomas ,and it provides some references for the judgement of histological type of primary lung cancer .%目的：探讨甲状腺转录因子-1（TTF-1）在肺癌组织中表达情况其临床诊断意义。方法采用免疫组织化学法分别检测50例原发性肺癌（腺癌23例、鳞癌14例、肺小细胞癌9例、肺大细胞癌4例）组织、癌旁组织及21例来源于胃肠道或卵巢转移性肺腺癌组织标本中TTF-1的表达情况，并分析TTF-1在肺癌诊断与鉴别诊断中的意义。结果腺癌、鳞癌、肺小细胞癌、肺大细胞癌、转移性肺腺癌组织TTF-1表达阳性率分别为73．9％（17／23）、0、77．8％（7／9）、50．0％（2／4）、0，腺癌、肺小细胞癌组织 TTF-1表达阳性率较鳞癌高（ P＜0．05）。原发肺癌的癌旁组织中，正常或增生肺泡上皮大部分不着色或较淡，阳性细胞少（＜20％）。继发性肺癌的癌旁组织中TTF-1表达均为阴性。肺癌组织TTF-1表达阳性率与患者年龄、性别、肿瘤大小、肿瘤部位均无明显关系（ P＞0．05）。结论 TTF-1可作为一个鉴别原发性或转移性肺腺癌的理想指标，并且TTF-1在原发性肺癌的组织类型判断上也能提供一定参考。
