血管狭窄性脑缺血患者血浆半乳糖凝聚素-3水平及其与血管狭窄程度及预后的关系

摘要

目的 探讨血管狭窄性脑缺血患者血浆半乳糖凝聚素-3(galectin-3)水平及其与血管狭窄程度及患者预后的关系.方法 将86例血管狭窄性脑缺血患者(观察组)按血管狭窄程度分成轻、中、重度狭窄组,选择同期30名健康志愿者为对照组,检测并比较各组血浆galectin-3表达水平,采用改良Rankin残障量表(mRS)对86例患者脑缺血预后进行评估.分析观察组患者galectin-3水平与脑血管狭窄程度、脑缺血预后评分的相关性.采用受试者工作特征(ROC)曲线评估galectin-3对脑缺血患者预后的预测价值.结果 观察组血浆galectin-3水平高于对照组(P＜0.05).轻、中、重度狭窄组血浆galectin-3水平依次升高(P＜0.05).脑缺血患者预后mRS评分＜3分者血浆galectin-3水平低于评分≥3分者(P＜0.05),且患者血浆galectin-3水平与脑血管狭窄程度及预后mRS评分均呈正相关(P＜0.05).血浆galectin-3水平预测脑缺血预后的ROC曲线下面积为0.756(P ＜0.001).结论 Galectin-3可能参与了血管狭窄性脑缺血的发生及发展,其对血管狭窄性脑缺血患者的预后有一定预测价值.%Objective To investigate the relationship of plasma galectin-3 level with angiostenosis severity and prognosis in patients with angiostenosis-related cerebral ischemia.Methods Eighty-six patients (observation group) with cerebral ischemia due to angiostenosis were divided into mild,moderate and severe stenosis groups according to the severity of angiostenosis.Concurrent 30 healthy volunteers were enrolled in the control group.The expression levels of plasma galectin-3 were detected and compared among all groups.The modified Rankin Handicap Scale(mRS) was used to evaluate the prognosis of the 86 patients with cerebral ischemia.The correlation of galectin-3 level with the severity of cerebral angiostenosis or the prognosis score of cerebral ischemia was analyzed in the observation group.Receiver operating characteristic (ROC) curve was used to evaluate the value of galectin-3 in predicting the prognosis of patients with cerebral ischemia.Results The level of plasma galectin-3 in the observation group was higher than that in the control group (P ＜ 0.05).The plasma galectin-3 levels increased in the order of the mild,moderate and severe stenosis groups (P ＜ 0.05).Among patients with cerebral ischemia,the plasma galectin-3 level of patients with mRS score ＜ 3 was lower than that of patients with mRS score ＞ or =3 (P ＜ 0.05),and the plasma galectin-3 level positively correlated with angiostenosis severity and mRS score of prognosis (P ＜ 0.05).The area of ROC curve of plasma galectin-3 level for predicting the prognosis of cerebral ischemia was 0.756(P ＜ 0.001).Conclusion Galectin-3 might involve in the occurrence and development of angiostenosis-related cerebral ischemia,and has a certain value in predicting the prognosis of patients with angiostenosis-related cerebral ischemia.
