穿孔性阑尾炎术后腹腔引流与非引流效果对比的Meta分析

摘要

目的 比较穿孔性阑尾炎术后腹腔引流与非引流的效果.方法 计算机检索PubMed、Cochrane Library、Embase、知网、中国生物医学数据库、维普及万方等数据库中关于穿孔性阑尾炎术后腹腔引流与非引流效果对比的随机对照试验.筛选文献后进行质量评价.采用RevMan 5.2软件进行Meta分析.结果 共纳入14篇文献,包括2493例患者.Meta分析结果显示:和非引流组相比,术后引流组肠粘连发生率较高(P<0.05),两组术后切口感染率及腹腔脓肿发生率比较,差异均无统计学意义(P>0.05).在儿童患者中,术后腹腔引流者住院天数多于非引流者(P<0.05).结论 穿孔性阑尾炎患者术后不进行腹腔引流更具优势.%Objective To compare the clinical efficacy between patients with versus without abdominal drainage after appendectomy for perforated appendicitis. Methods A computer-based retrieval was performed in the databases, such as PubMed, Cochrane Library, Embase,CNKI,CBM,VIP and WanFang Data for the randomized controlled trials about the efficacy comparison between patients with versus without abdominal drainage after appendectomy for perforated appendicitis. A quality assessment was conducted in the selected literatures. RevMan 5. 2 software was applied to a Meta-analysis. Results A total of 14 trials involving 2493 patients were included. The results of the Meta-analysis showed that the postoperative drainage group had a higher incidence rate of intestinal adhesion compared to the non-drainage group(P<0. 05),and no statistically significant differences were found in the incidence rate of postoperative incision infection or abdominal abscess between the two groups(P>0. 05). The children with postoperative abdominal drainage got more hospital stay compared with the children without abdominal drainage(P <0. 05). Conclusion The patients with perforated appendicitis who do not receive abdominal drainage might be advantageous.
