免疫细胞化学EMA、CEA、CD68、Vimentin组合在胸腹水细胞块病理诊断中的应用研究

Objective To explore the values of combined application of immunocytochemistry markers EMA, CEA, CD68 and Vimentin in pathological diagnosis of cells from pleuroperitoneal effusion. Methods The cell mass of 189 cases from pleuroperi-toneal effusion were collected to stain immunocytochemistry markers of EMA, CEA, CD68 and Vimentin, the expression of each in-dex in glandular epithelial cells, mesothelial cells and histocytes was observed and their diagnostic values were investigated. Re-sults For adenocarcinoma cells, positive expression rate of EMA and CEA were 96.7% and 63.3% respectively; CD68 and Vi-mentin had no strong positive expression; but a few cases showed weak positive expression. For histocytes, the positive expression rate of CD68 was 84.7% and a few cases showed weak positive expression. For mesothelial cells, positive expression rate of Vi-mentin was 86.6% and a few cases showed weak positive expression. Conclusion Combined application of immunocytochemistry markers EMA, CEA, CD68 and Vimentin was facilitate the analysis of the histologic origin of cells from the pleuroperitoneal effu-sion, it was very useful for the diagnosis of adenocarcinoma.%目的：探讨免疫细胞化学标记EMA、CEA、CD68、Vimentin组合在胸腹水细胞病理诊断中的价值。方法收集189例胸腹水病例细胞块做免疫细胞化学EMA、CEA、CD68、Vimentin标记，观察各指标在腺上皮、间皮细胞、组织细胞上的表达情况，研究其诊断价值。结果腺癌细胞EMA96.7%阳性表达；CEA63.3%阳性表达；CD68与Vimentin没有明显阳性表达，少数病例有弱阳性表达。组织细胞CD68阳性表达率84.7%，少数病例弱阳性表达。间皮细胞Vimentin86.6%阳性表达，少数病例弱阳性表达。结论免疫细胞化学指标EMA、CEA、CD68、Vimentin组合可以帮助分析胸腹水细胞的组织来源，对腺癌的诊断很有帮助。
