2014-2016年武警安徽省总队医院住院患者肠外营养制剂应用分析

摘要

OBJECTIVE:To investigate the application of parenteral nutrition of inpatients in Anhui Provincial Corps Hospital of Chinese People`s Armed Police Forces(hereinafter referred to as "our hospital"), so as to provide reference for clinical rational drug application. METHODS:Data of usage and consumption sum of parenteral nutrition of inpatients of our hospital during 2014-2016 were extracted from information management system, DDDs, sequence of consumption sum (B)/sequence of DDDs(A) and DDC were calculated and analyzed. RESULTS:During 2014-2016, the consumption sum of parenteral nutrition and its constituent ratio increased year by year; the consumption sum of vitamins increased fast, and ranked the 1st in the three years; the consumption sum of compound amino acid injection (15-HBC), inverted sugar injection and various microelement injection (Ⅱ) increased year by year; the DDC of fat emulsion amino acid (17) glucose injection, inverted electrolyte injection, fructose injection and various microelement injection (Ⅱ) were higher than 100 yuan; the B/A of medium-long chain fat emulsion injection, alanyl-glutamine injection, fructose injection and various microelement injection (Ⅱ) were closed to 1. CONCLUSIONS:The application of parenteral nutrition of inpatients in our hospital is basically rational, clinicians and pharmacists should manage the indications of parenteral nutrition and reinforce the rationality of parenteral nutrition supporting.%目的:了解武警安徽省总队医院(以下简称"我院")住院患者肠外营养制剂应用情况,为临床合理用药提供依据.方法:从医院信息管理系统中提取2014-2016年我院住院患者肠外营养制剂的使用量、销售金额等数据,统计分析药品的用药频度(defined daily dose system,DDDs)、销售金额排序(B)/DDDs排序(A)、限定日费用(defined daily cost,DDC).结果:2014-2016年,我院肠外营养制剂的销售金额及其构成比逐年增长;维生素类药物的销售金额增长较快,3年来其销售金额排序均居首位;复方氨基酸注射液15-HBC、转化糖注射液、多种微量元素注射液(Ⅱ)的销售金额逐年增长;脂肪乳氨基酸(17)葡萄糖(11%)注射液、转化糖电解质注射液、果糖注射液、多种微量元素注射液(Ⅱ)的DDC均>100元;中长链脂肪乳注射液、丙氨酰谷氨酰胺注射液、果糖注射液、多种微量元素注射液(Ⅱ)的B/A均接近于1.结论:我院住院患者肠外营养制剂的使用基本合理,临床医师与药师需共同努力,掌握肠外营养制剂的适应证,加强肠外营养支持的合理性.
