硫酸镁治疗尖端扭转型室性心动过速31例疗效分析

摘要

ObjectiveTo discuss the mechanism and curative effect of magnesium sulfate in the treatment of torsade de pointes (Tdp).MethodsRetrospective analysis of 31 cases of clinical data with Tdp from 2005 January to 2013 june in our hospital,besides the active control of primary disease and eliminate the cause of treatment,with 25% magnesium sulfate injection 10 ml diluted to 20~30 mL after intravenous injection,followed by 0.9% sodium chloride injection for 250 mL+ 25% magnesium sulfate 20 mL intravenous drip.ResultsAfter treatment all patients with TdP were controlled,no arrhythmia aggravation,the drop in blood pressure,respiratory depression,altered consciousness and renal function damage and other side-effect were occured.ConclusionTorsade de pointes is a clinical emergency situation, should actively control the primary disease and eliminate incentives;magnesium sulfate intravenous administration are the main measures, magnesium sulfate can be effective in the treatment of TdP,there is no obvious adverse reaction.%目的：探讨硫酸镁对尖端扭转型室性心动过速（Tdp）的治疗机制和疗效。方法回顾性分析2013年6月～2005年1月我院收治的尖端扭转型室性心动过速患者31例的临床资料，给予患者积极控制原发疾病和消除诱因治疗，25%硫酸镁10 mL和0.9%氯化钠注射液稀释至20～30 mL后静脉推注，继以0.9%氯化钠注射液250 mL+25%硫酸镁20 mL静脉滴注维持。结果用药后所有患者TdP得到控制，未出现心律失常加重、血压下降、呼吸抑制、肾功能损害及神志改变等副反应。结论尖端扭转型室速是临床危急情况，应积极治疗原发疾病和纠正诱因；静脉使用硫酸镁是主要措施，硫酸镁能有效治疗TdP，并且无明显副反应发生。
