2型糖尿病患者血β-羟丁酸水平与空腹血糖及糖化血红蛋白的关系

             

摘要

目的:探讨2型糖尿病患者血β-羟丁酸(HBA)水平与空腹血糖(FBG)及糖化血红蛋白(HbA1c)的关系。方法选择该院门诊及住院的2型糖尿病患者287例,测定血HBA、FBG、HbA1c值。根据FBG水平分为I组( FBG<4 mmol/L)、II组(4 mmol/L≤FBG<7 mmol/L)、III组(7 mmol/L≤FBG<10 mmol/L)、IV组(10 mmol/L≤FBG<13 mmol/L)、V组(FBG≥13 mmol/L);根据HbA1c水平分为A组( HbA1c<7%)、B组(7%≤HbA1c<9%)、C组( HbA1c≥9%),进行相关的统计学分析。结果FBG≥13mmol/L组血HBA水平最高,但各组间差异无统计学意义( P>0.05)。 HbA1c≥9%组血HBA水平明显高于HbA1c<7%组及7%≤HbA1c<9%组,差异有统计学意义(P<0.05)。结论2型糖尿病患者血β-羟丁酸水平与空腹血糖无明显相关性。 HbA1c≥9%的患者应注意监测血β-羟丁酸水平,积极控制血糖,防治糖尿病酮症或酮症酸中毒。%Objective To analyze and discuss the relationship between the level of blood beta-hydroxybutyrate(HBA) and fasting blood glucose(FBG), and glycated hemoglobin(HbA1c) in type 2 diabetic(T2DM) patients.Methods 287 cases of T2DM pa-tients were chosen as subjects. HBA, FBG and HbA1c were determined in all patients. According to measured FBG levels, these patients were divided into group I (FBG<4 mmol/L), group II (4 mmol/L≤FBG<7 mmol/L), group III (7 mmol/L≤FBG<10 mmol/L) , group IV ( 10 mmol/L≤FBG<13 mmol/L) , group V ( FBG≥13 mmol/L) . Also according to measured HbA1c levels, all these patients were divided into group A ( HbA1c<7%) , group B ( 7%≤HbA1c<9%) , group C ( HbA1c≥9%) . The statisti-cal analysis was made.Results The level of HBA in patients with FBG≥13 mmol/L was the highest, but the difference between groups were not statistically significant ( P>0.05) . The level of HBA in patients with HbA1c≥9% was significantly higher than the other two groups (P<0.05). Conclusion The HBA level of T2DM patients was not relevant with FBG. Patients with HbA1c≥9% should monitor their HBA level. Blood glucose should be strictly controlled to prevent the occurrence of ketosis or ketoacido-sis.
