18F-FDG PET/CT显像诊断结外淋巴瘤的应用价值

摘要

Objective To explore the clinical application values of PET / CT diagnosis of extranodal lymphoma. Methods 18F-FDG PET/CT scanning was performed on 42 patients with lymphoma using the GE Discovery LS PET/CT Scanner of (America). The metabolism imaging results of 15 primary and/or secondary lymphoma patients were qualitatively and semi-quatitatively analysed and compared with that of CT scanning, the results of which was verifed by pathology. Results PET/CT metabolism imaging outcome show 20 of 18F-FDG highly uptake focuses in 15 lymphoma patients.The sensitivity, specificity and positive rate of PET/CT on extranodal lymphoma focuses were 90%, 100%, and 91%, repectively. CT Imaging results displayed 9 true positive focuses and 2 ture negative focuses, the sensitivity, specificity and positive rate of CT were 35%, 100% and 41%, respectively. The conformity between CT and PET/CT inspection was 45%.Conclusion PET/CT 18F-FDG metabolism imaging has great clinical diagnosis value on extranodal lymphoma.%目的 研究PET/CT诊断结外淋巴瘤的临床应用价值.方法 应用GE Discovery LS PET/CT扫描机,对42例淋巴瘤患者进行18F-FDG代谢显像,对其中15 例(非霍奇金淋巴瘤 non Hodgkin lympooma,NHL,13例,霍奇金淋巴瘤Hodgkin lympooma,HL,2例)原发和/或继发结外淋巴瘤的显像结果进行定性和半定量分析,并与CT扫描结果进行比较,病理学诊断为金标准.结果 PET/CT代谢显像发现15例结外淋巴瘤中20个18 F-FDG摄取异常增高病灶,其对结外淋巴瘤病灶的探测的PET/CT显像灵敏度、特异性、准确率分别为90%、100%、91%;CT显像检出9个真阳性病灶, 2个真阴性病灶,灵敏度、特异性、准确率分别为35%、100%、41%.CT与PET/ CT检查的一致性为45%.结论 PET/CT 18F-FDG代谢显像对结外淋巴瘤具有重要的临床诊断价值.
