目的探讨Parks手术结合结肠成形在低位直肠癌保肛手术中的应用价值.方法回顾性分析2009年2月~2011年10月在山东省青州荣军医院外科行直肠癌保肛手术患者64例临床资料.结果本组病例均获得随访,其中术后发生吻合口瘘3例(4.7%),吻合口狭窄2例(3.1%),出现局部复发1例(1.6%)；术后6个月统计每日排便次数：1~2次/d 48例,占75.0%,3~4次/d 13例,占20.3%,>4次3例,占4.7%；术后12个月统计每日排便次数：1~2次/d 61例,占95.3%,3~4次/d 3例,占4.7%.患者手术前后的比较差异有统计学意义(P<0.05).结论应用经肛门结肠肛管吻合(Parks手术)结合结肠成形术能重新建立患者良好贮粪功能,明显改善患者保肛术后的排便功能,较单纯肛管吻合术优越性明显,是低位直肠癌保肛手术的首选术式.% Objective To investigate the Parks surgery combined with colon forming in rectal cancer surgery. Methods A retrospective analysis of 64 cases in the hospital 2 lines of rectal cancer surgery in February 2009 to October 2011 clinical data. Results The patients were followed up. Among them, the postoperative anastomosis fistula in 3 cases (4.7%), anastomotic stenosis in 2 cases (3.1%) had local recurrence one cases (1.6%);after 6 month statistics daily number of bowel movements 1~2 times / d 48 cases, accounting for 75.0%,3~4 times / d 13 cases, accounting for 20.3%,>4 times and 3 cases (4.7%);12 months after operation statistics of daily number of bowel movements 1~2 times / d61 cases, accounting for 95.3%,3~4 times / d3, cases, accounting for 4.7%. Patients before and after difference was statistically significant (P<0.05). Conclusion Transanal coloanal anastomosis (Parks operation) combined with colon forming anal anastomosis surgery to re-establish good storage in patients with fecal function, significant improvement in anal sphincter preservation in patients with postoperative bowel function than a simple superiority obvious. It is the preferred technique for low rectal cancer surgery.
