老年急性心力衰竭患者诊断指标的临床观察

摘要

Objective To investigate the clinical significance of diagnosis index in elderly patients with acute heart failure. Methods from 2010 January to 2012 January in 70 cases of senile heart failure patients in our hospital as the experimental group, elderly patients at the same time were selected in our hospital 70 cases of the Department of internal medicine as control group,two groups of patients in the aspects of gender,age,no significant difference, comparable.Compared to experimental group and control group, the data in patients with cardiac trooping testing content,at the same time,through the determination of creative kinas is enzyme MB index approach, detection of myocardial trooping index sensitivity and specificity level.The above parameters to obtain at admission were within half an hour. Results The experimental group of cardiac trooping assay for (0.15±0.12) ng/L, the control group of patients with myocardial trooping was (0.011±0.003)ng/L,the experimental group was significantly higher than that of the control group, significant difference between the two groups(P<0.05).At the same time,the experimental group of myocardial trooping detection sensitivity was 84.2%, specificity was 88.9%,the detection sensitivity of creative kinas is enzyme MB index was 57.9%,the specificity was 85.6%, the detection sensitivity of cardiac trooping T was significantly higher than that of creative kinas MB is enzyme, compared two groups,significant difference(P<0.05). Conclusion The experimental indexes of heart failure combined with clinical diagnosis in favor of emergency quickly and accurately diagnosed in elderly patients with heart failure.%目的：探讨老年急性心力衰竭患者诊断指标的临床意义。方法选取2010年1月-2012年1月遵义医学院第五附属医院收治的70例老年心力衰竭患者作为实验组，选取同期入院的70例各内科的老年患者作为对照组。针对两组患者测定心肌肌钙蛋白含量的数据进行对比，同时通过测定肌酸激酶MB同功酶指标的方式，检测心肌肌钙蛋白指标的灵敏度以及特异性水平。上述各项指标要在患者入院就诊的30 min内获得。结果实验组患者心肌肌钙蛋白含量测定为（0.15±0.12）ng/L，对照组患者心肌肌钙蛋白含量测定为（0.011±0.003）ng/L，实验组明显高于对照组，两组相比差异显著（P＜0.05）。同时，实验组心肌肌钙蛋白含量检测灵敏度为84.2%，特异度为88.9%，肌酸激酶MB同功酶指标检测灵敏度为57.9%，特异度为85.6%，心肌肌钙蛋白含量检测灵敏度明显高于肌酸激酶MB同功酶，两组相比差异显著（P＜0.05）。结论心力衰竭的各项实验指标结合临床诊断有利于在急诊中准确快速地确诊老年患者心力衰竭的病症。
