肠内与肠外营养对食管癌术后恢复的影响

     

摘要

目的比较肠内营养(enteral nutrition,EN)与肠外营养(parenteral nutrition,PN)对食管癌患者术后恢复的影响。方法统计合肥市第三人民医院胸外科行食管癌根治术EN 26例与PN 25例患者的临床资料,比较两组间血常规部分指标、生化部分指标、转铁蛋白(transferrin,TF)、视黄醇结合蛋白(retinol binding protein,RBP)、并发症率、病死率等指标是否有差异。结果术前各指标未见明显统计学差异(P>0.05)。术后第10天血常规部分指标、生化部分指标、TF、RBP等无明显统计学差异(P>0.05)。术后感染率,吻合口瘘等均无统计学差异(P>0.05)。而白细胞计数、胃肠功能恢复时间、住院费用、血糖及电解质紊乱次数有统计学差异(P<0.05)。结论食管癌术后EN对患者营养状况的恢复及并发症的发生率较PN并未显示出明显的优势,但在胃肠功能恢复、住院费用、血糖及电解质控制方面优于PN组患者。%Objective The aim of this study was to compare the inlfuence of nteral nutrition for esophageal carcinoma patients’ postoperative recovery with parenteral nutrition. Methods We studied 26 patients with nteral nutrition and 25 patients with parenteral nutrition after esophagectomy in our department. Part indexes of blood routine test、biochemistry、transferrin、retinol binding protein、complication rate、mortality of patients are compared between two groups. Results There was no obvious difference in preoperative and postoperation’s blood routine test、biochemistry、transferring、retinol binding protein between two groups(P>0.05). But nteral nutrition signiifcantly decreased the gastrointestinal functional recovery time、hospitalization costs、blood sugar and electrolyte imbalance times of patients in contrast to parenteral nutrition group(P<0.05). Conclusion These ifndings shows that nteral nutrition would be beneifcial to esophageal carcinoma patients’ postoperative recovery as compared with parenteral nutrition.
