培美曲塞单药或联合卡铂二线治疗晚期非小细胞肺癌疾病控制率及粒细胞减少的Meta分析

             

摘要

目的:对国内外晚期非小细胞肺癌经培美曲塞单独用药或联合卡铂二线治疗的相关文献进行 Meta分析,分析其对疾病控制率、粒细胞减少的影响。方法检索中国知网、Pubmed等学术文献资源库,收集2009―2011年国内外公开发表的有关培美曲塞单独用药或联合卡铂二线治疗晚期非小细胞肺癌疗效比较的文献,文献异质性检验结果用随机效应模型或固定效应模型进行合并分析;敏感性分析不同纳入标准的结果,漏斗图用于评估发表的偏倚。结果根据所收集文献资料特点,对两种治疗方案进行敏感性分析,结果显示可靠性和稳定性均较高。使用漏斗图对资料进行分析,结果显示漏斗图基本对称,发表性偏倚得到有效控制。结论1.培美曲塞单独用药或联合卡铂二线治疗均是晚期非小细胞肺癌的有效治疗方案,其控制率的差异无统计学意义( P>0.05);2.培美曲塞单用药治疗对粒细胞计数影响较小;3.敏感性分析与发表性偏倚识别结果显示本次Meta分析的结果稳定,可靠。%Objective To analyze the literature related data at domestic and overseas of pemetrexed monotherapy and pemetrexed combined with carboplatin on advanced non-small-cell lung cancer with Meta-analysis, and analyze the influence of treatment methods on diseases control rate and granulocytopenia. Methods Retrieval CNKI, Pubmed and other academic literature database was used to collect the clinical literatures published in domestic and overseas form 2009 to 2011 on pemetrexed drug alone use or combined with carboplatin in the second-line treatment of advanced non small cell lung cancer (NSCLC). Literature heterogeneity test results suggested that random effects model or fixed effect model was used to analyze the results of the sensitivity analysis, and the funnel plot was used to evaluate the publication bias. Results According to the character of the literatures, sensitivity analysis of two groups impled that the stable and reliable control rate were both reach high scheme. The funnel plots of the two risk factors were all symmetrical which indicates that the publication bias under control. Conclusion 1.Diseases control rate has no difference between the pemetrexed monotherapy and pemetrexaed combined with carboplatin second-line treatment for advanced non-small cell lung cancer; 2.the pemetrexed monotherapy has less influence on granulocytopenia adverse-effect; 3.the two sensitivity analysis results and publication bias under control prove that the conclusion of this research work is stable and reliable.
