目的 探讨喉癌部分喉切除术后复发患者的有效治疗方法.方法 回顾分析我院1999年1月～2005年10月收治的喉癌部分喉切除术后肿瘤复发并行挽救治疗的26例临床资料.结果 26例首次术后至复发时间平均22个月,均行挽救治疗,术式包括全喉切除、根治性颈淋巴结清扫、支撑喉镜复发癌局部切除、喉次全切除加选择性颈淋巴结清扫、发声重建术等,11例同时行术后放疗,3例单纯行放疗.治疗后有4例再复发,均行第3次手术,其中1例行第4次手术.随访5年,挽救治疗后总体3年生存率57.69%(15/26),5年生存率34.62%(9/26).结论 喉癌部分喉切除术后复发患者以局部复发为主,目前挽救性治疗仍以全喉切除为主,患者预后差.%Objective To investigate the effective treatment for laryngeal carcinoma recurrence after partial laryngectomy.Methods 26 patients with laryngeal carcinoma recurrence after partial laryngectomy were admitted to our hospital from January 1999 to October 2005, and the clinical data of the patients were retrospectively analyzed.Results The average time from the first surgery to recurrence was 22 months, All the 26 patients underwent salvage therapy, including total laryngectomy, radical dissections of neck lymph nodes, local excision of recurrent lesion with supporting laryngoscopes, subtotal ectomy with radical dissections of neck lymph nodes and voice rehabilitation, et al.11 patients underwent the radiotherapy after surgery, and the other 3 patients only underwent radiotherapy.4 patients with recurrence after the salvage therapy underwent the 3rd operation, among whom, 1 even had the fourth surgery.All the patients were followed up for 5 years, and the total 3-year and 5-year survival rate in patients with salvage treatment after recurrence was 57.69 ％ ( 15/26), and 34.62％ (9/26) respectively.Conclusion Local recurrence is the main cause of laryngeal carcinoma recurrence after partial laryngectomy, and the preferred treatment of salvage therapy is total laryngectomy, but the prognosis is poor.
