原发性干燥综合征患者唇腺中热休克蛋白60与细胞外基质金属蛋白酶诱导因子的表达及意义

     

摘要

目的 探讨热休克蛋白60 (heat shock protein 60,HSP60)、细胞外基质金属蛋白酶诱导因子(extracellular matrix metalloproteinase inducer,CD147)在原发性干燥综合征(primary Sj(o)gren syndrome,PSS)发病过程中的作用及意义.方法 采用免疫组织化学染色链霉菌抗生物素-蛋白过氧化酶法(SP法)对52例PSS患者、30例正常对照组唇腺中HSP60 、CD147表达进行观察,并对二者的表达进行相关性分析.结果 PSS组HSP60均阳性高表达,导管区表达面积PSS组2 585.96(1 053.18) μm2 vs正常对照组1013.50(711.25) μm2,表达光密度PSS组0.25(0.11) vs正常对照组0.10(0.01)(P<0.01),腺泡区表达面积PSS组2 0267.50(8 327.75) μm2 vs正常对照组6 419.00(720.00) μm2,表达光密度PSS组0.172(0.078) vs正常对照组0.074(0.01)(P<0.01),HSP60表达随淋巴细胞浸润灶数的增加而表达增强.CD147在PSS组均阳性高表达,导管区表达水平,面积PSS组2 990.50(1 172.75) μm2 vs正常对照组982.00(162.00) μm2,表达光密度PSS组0.200(0.19) vs正常对照组0.10(0.00)(P<0.01),腺泡区表达面积PSS组15 980.00(14 444.00) μm2 vs正常对照组7 873.00(6 057.00)μm2,表达光密度PSS组0.14(0.07) vs正常对照组0.80(0.02)(P<0.01),CD147表达随淋巴细胞浸润灶数的增加而表达增强.HSP与CD147的表达呈正相关(P<0.01).结论 HSP60、CD147在PSS患者唇腺组织中异常表达,均参与PSS的发病过程.%Objective To investigate the role of heat shock protein 60CHSP60) and extracellular matrix metalloproteinase inducer (CD147) in primary Sjogren syndrome (PSS) pathogenesis and its clinical significance. Methods Biopsies of minor labial salivary glands were obtained from 52 cases of patients with PSS and 30 cases of healthy controls. The streptavidin immunohistochemical staining was used to detect the expression and location of HSP60 and CD147 in labial salivary glands. Quantitative analysis was performed by image analysis software-image plus 5. 0 at the site of positive expression of HSP60 and CD147,the correlation between the expression of HSP60 and CD147 was calculated by SPSS 13. 0 software in the salivary glands of patients with PSS. Results HSP60 expression level of area and optical density in patients with PSS were significantly higher than those of healthy controls in the ductal region (area:2 585.96(1 053.18) μm2 vs 1013.50(711.25) μm ,optical density: 0.25(0.11) vs 0.098(0. 01)( P <0. 01). The HSP60 expression level of area and optical density in patients with PSS were significantly higher than those of healthy controls in the acinic region (area:2 0267. 50(8327. 75) μm2 vs 6 419. 00(720. 00) μm2 .optical density: (0. 17± 0. 08) vs 0. 07(0. 01) ( P <0. 01). The CD147 expression level of area and optical density in patients with PSS were significantly higher than those of healthy controls in the ductal region ( area: 2 990.50(1 172.75) μm2 vs 982.00 (162. 00) μm2 .optical density:0. 20(0. 19) vs 0. 10(0. 00)( P <0. 01). The CD147 expression level of area and optical density in patients with PSS were significantly higher than those of healthy controls in the acinic region (area: 15 980.00(14 444.00) μm2 vs 7873.00(6 057.00) μm2 .optical density :0. 14(0.07) vs 0.08(0. 02) ( P <0. 01). There were significant positive increase between lymphocyte foci score and expression upregulation of HSP60 and CD147 in labial salivary glands from 52 cases of patients with PSS( P <0. 01). There was linear positive correlation between expression of HSP60 and CD147 in the salivary glands of patients with PSS( P <0. 01 or <0. 05). ConclusionThe results of this study suggest that the abnormal expression of HSP60 and CD147 are involved in the pathogenesis of PSS and play a crucial role. The interaction of HSP60 and CD147 may be one of the important mechanism leading to the labial salivary glands destruction found in PSS.
