住院脑卒中患者抑郁障碍现况调查

Objective: To explore the prevalence of depressive disorder in stroke patients in hospital, and to provide reference for early recognition and special intervention for post-stroke depression patients. Methods: As a screening and diagnostic tool, the Comprehensive International diagnostic Interview, core version 3.0 (CIDI-3.0) was used to evaluate 405 stroke patients in hospital, including 80 minority nationality patients. The patients were sampled from the neurology departments of three general hospitals. Among them, 202 were from an urban hospital and 203 were from rural areas. All of them were diagnosed as stroke by Computed Tomography (CT) or Magnetic Resonance Imaging (MRI). About 70% of them had their first episode of stroke and were in their first year of stroke episode. Results: The screening positive rate of depressive disorder in stroke patients was 26. 2%, and the positive rate of diagnosis was 5.4%. The positive rates of diagnosis in minority nationalities (15. 0% vs. 3. 1 %), rural dwellers (8.2% vs. 1.7%) and manual workers (7. 8% vs. 2.3%) were higher than Han nationality, urban dwellers and non-manual workers, respectively (Ps <0.05). Conclusion: In this study, the prevalence of depressive disorder in stroke patients in hospital is 5.4%. It is 15.0% in minority nationalities, which is higher than in Han nationality. The risk of depressive disorder in stroke patients may be associated with nationality, residence and occupation.%目的:调查住院脑卒中患者抑郁障碍的患病情况,为卒中后抑郁的早期识别和针对性干预提供参考.方法:在城乡综合性医院神经内科抽取405例住院脑卒中患者,其中含少数民族80例,应用复合性国际诊断问卷-3.0 (CIDI-3.0)中文版作为筛查和诊断工具进行测评.结果:脑卒中患者抑郁障碍的筛查阳性率为26.2％,诊断阳性率为5.4％.少数民族(15.0％vs.3.1％)、农村居民(8.2％vs.1.7％)和体力劳动者(7.8％vs.2.3％)中诊断阳性率高于汉族、城市居民和非体力劳动者(均P＜0.05).结论:住院脑卒中患者抑郁障碍的患病率为5.4％,少数民族患病率为15.0％,脑卒中后抑郁障碍的风险因素中应重视民族、居住地及职业等相关因素.
