单球囊与欣普贝生用于足月引产的临床效果

摘要

Objective To compare the efficacy and safety of domestic single balloon ( disposable balloon cervical dilator ) and Dinoprostone Suppositories in promoting cervical ripening and inducing labor in full-term pregnant women .Methods A total of 200 full-term pregnant women who had medical indication of labor induction and whose cervical Bishop scores were lower or equal to 6 were randomly divided into single balloon group ( n =100 ) and Dinoprostone Suppositories group ( n =100 ) .All these women had received treatment at Taizhou Hospital of Zhejiang Province .Women in the single balloon group were given domestic balloon in combination with small dose of oxytocin intravenously to induce labor .As for patients in the Dinoprostone Suppositories group , they received Dinoprostone Suppositories by posterior fornix administration .A retrospective analysis was carried out to compare the effects of single balloon and Dinoprostone Suppositories in promoting cervical ripening and inducing labor as well as the incidence rate of complications .Results The total effective rate of promoting cervical ripening reached 98% in the single balloon group , which was significantly higher than 87% in the Dinoprostone Suppositories group (χ2 =8.721,P<0.05).The cesarean section rate was 15%in the single balloon group, which was remarkably lower than 33%in the Dinoprostone Suppositories group (χ2 =8.882,P<0.05).The incidence rate of complications was 8% in the single balloon group, which was significantly lower than 29% in the Dinoprostone Suppositories group (χ2 =16.624,P<0.05).Conclusion Compared with Dinoprostone Suppositories , domestic single balloon in combination with intravenous drip of small dose of oxytocin can achieve better effects in promoting cervical ripening and inducing labor in full-term pregnant women .Besides , in terms of complications , domestic single balloon is far better than Dinoprostone Suppositorie .Therefore , domestic single balloon is worth popularizing in clinics .%目的：比较国产单球囊（一次性球囊宫颈扩张器）与欣普贝生用于足月妊娠促宫颈成熟的及引产效果及有效性和安全性。方法将在浙江省台州医院就诊的200例有医学引产指征，且宫颈Bishop评分≤6分的足月妊娠的孕妇，按照抽签方法随机均分为单球囊组（100例）和欣普贝生组（100例）。单球囊组给予国产单球囊联合小剂量催产素静脉点滴引产，欣普贝生组以欣普贝生阴道后穹窿给药引产。回顾性分析两组促宫颈成熟及引产效果、并发症发生率等情况。结果①单球囊组促宫颈成熟总有效率为98％，显著高于欣普贝生组的87％，χ2＝8．721，P＜0．05；②单球囊组剖宫产率为15％，显著低于欣普贝生组的33％，χ2＝8．882，P＜0．05；③单球囊组总并发症发生率为8％，显著低于欣普贝生组的29％，χ2＝16．624，P＜0．05。结论与欣普贝生相比，国产单球囊联合小剂量催产素静脉点滴引产用于足月妊娠促宫颈成熟及引产的效果更为显著，在并发症方面显著低于欣普贝生，值得在临床上加以推广及应用。
