不同浓度盐水雾化吸入对毛细支气管炎患儿治疗效果的对比

             

摘要

目的 探讨不同浓度氯化钠溶液雾化吸入对毛细支气管炎患儿治疗的效果对比.方法 收集2010年2月至2014年8月在南通大学附属海安医院确诊为毛细支气管炎的82例患儿作为研究对象,应用随机数字表法分为A、B、C、D、E共5组,每组均采用复方异丙托溴铵气雾剂1.25mL、布地奈德混悬液1mL雾化吸入(2次/d),之后A组雾化吸入注射用水,B组雾化吸入0.9%氯化钠溶液,C组雾化吸入3.0%氯化钠溶液,D组雾化吸入5.0%氯化钠溶液,E组雾化吸入10%氯化钠溶液,对五组患儿雾化吸入治疗前后进行临床严重度评分,比较5组患儿住院时间和睡眠质量(夜间睡眠时间、夜醒次数),观察治疗后患儿临床症状改善情况.结果 在睡眠时间、夜醒次数、平均住院时间和临床严重度评分差异上,A组与B组差异均无统计学意义(t值分别为1.282、1.639、0.829、0.945,均P>0.05);A、B、C、D、E组5组之间比较均具有统计学意义(F值分别为3.925、5.241、5.103、5.131,均P<0.05);C组与D、E两组比较均具有统计学意义(tC-D值分别为1.994、2.247、2.733、2.843;tC-E值分别为2.057、2.213、2.436、2.747,均P<0.05);D、E两组比较差异均无统计学意义(t值分别为1.132、1.589、0.737、0.833,均P>0.05).结论 一定高浓度的盐水雾化吸入配合治疗能显著改善毛细支气管患儿的疾病状况.%Objective To investigate and compare the therapeutic effects of different concentrations of aerosolized saline inhalation in the treatment of children with capillary bronchitis.Methods A total of 82 cases of children diagnosed with capillary bronchitis at Hai' an Hospital Affiliated to Nantong University from February 2010 to August 2014 were chosen as research objects.In accordance with the random number table, they were divided into five groups, namely group A, B, C, D and E.Children in each group received 1.25mL of compound Ipratropium Bromide Aerosol and 1mL aerosolized inhalation of budesonide suspension ( twice per day) , and then, children in group A received water for injection through aerosolized inhalation.As for children in group B, group C, group D and group E, they received 0.9%, 3.0%, 5.0%and 10%sodium chloride solution through aerosolized inhalation, respectively.Clinical severity scores of all five groups of children were recorded before and after they receiving aerosolized inhalation treatment.Besides, duration of hospital stays and sleep quality ( sleeping hours and wake up times at night) of all those children were compared.In addition, improvements of their symptoms after treatment were also observed.Results There was no statistically significant difference in sleeping hours, wake up times, average duration of hospital stays and clinical severity score between children in group A and group B (t value was 1.282, 1.639, 0.829 and 0.945, respectively, all P>0.05).When comparisons were carried out among groups, the differences were of statistical significance (F value was 3.925, 5.241, 5.103 and 5.131, respectively, all P<0.05).When comparisons were carried out between group D and group E with group C, the differences were of statistical significance (tC-D value was 1.994, 2.247, 2.733 and 2.843, respectively;tC-E value was 2.057, 2.213, 2.436 and 2.747, respectively, all P<0.05).The difference between group D and group E had no statistical significance (t value was 113.2, 1.589, 0.737 and 0.833, respectively, all P>0.05).Conclusion Combined with other treatments, a certain volume of high concentration of aerosol saline inhalation can significantly improve the disease condition of children with capillary bronchitis.
