HCDP产妇的早产儿血常规和凝血功能特点及并发症分析

摘要

目的 分析重度妊娠高血压疾病(HCDP)产妇所产早产儿血常规和凝血功能特点及并发症的发生情况.方法 选取2015年1月至2017年6月就诊于首都医科大学附属北京世纪坛医院的HCDP产妇所产的早产儿50例为研究组,健康产妇所产早产儿50例为对照组.比较两组早产儿血常规、凝血功能的情况,并比较两组早产儿常见并发症的发生情况.结果 研究组血小板与白细胞总数较对照组均明显降低(t值分别为5.65、4.52,均 P<0.05);研究组纤维蛋白原(Fib)的水平较对照组明显降低(t=3.24,P<0.05),凝血酶原时间(PT)、血浆凝血酶时间(TT)及部分活化凝血酶原时间(APTT)较对照组均明显升高(t值分别为2.35、3.53、4.85,均 P<0.05).研究组感染、颅内出血及消化道出血的发生率较对照组明显升高(χ2值分别为8.85、7.54、7.43,均 P<0.05),而死亡率、呼吸窘迫综合征及支气管 -肺发育不良发生率与对照组比较,均无明显差异(χ2值分别为0.32、0.79、0.26,均 P>0.05).结论 相比健康产妇所产的早产儿,HCDP产妇所产的早产儿更容易出现血小板、白细胞总数减少和凝血功能异常,同时其感染、颅内出血及消化道出血的发生率较高,应引起临床重视.%Objective To analyze the characteristics of routine blood,coagulation function and complications in preterm infants produced by hypertensive disorders in pregnancy(HCDP)mothers.Methods Fifty cases of preterm infants produced by HCDP mothers in Beijing Shijitan Hospital were selected as case group,and 50 premature infants produced by healthy women as control group.The blood routine and coagulation function were compared between two groups,and the common complications were also compared.Results The platelet and white blood cell counts in the study group were significantly lower than those in the control group(t value was 5.65,4.52,respectively both P<0.05),and the fibrinogen(Fib)level in the study group was significantly lower than that of the control group(t=3.24,P< 0.05).Prothrombin time(PT),thrombin time(TT)and activated partial thromboplastin time(APTT)were significantly increased in comparison of the control group(t value was 2.35,3.53 and 4.85, respectively.all P<0.05).The incidence rates of infection,cerebral hemorrhage and gastrointestinal bleeding in the study group were significantly higher than those in the control group(χ2value was 8.85,7.54 and 7.43,respectively,all P<0.05),but the incidence rates of mortality,respiratory distress syndrome and bronchopulmonary dysplasia showed no significant differences in comparison of the control group(χ2value was 0.32,0.79 and 0.26,respectively,all P>0.05).Conclusion Compared to those premature infants produced by healthy puerperal,preterm infants produced by HCDP mothers are more likely to have lower platelet and white blood cell counts and coagulation dysfunction with higher incidence rate of infection,cerebral hemorrhage and gastrointestinal bleeding,so more attention should be paid.
