介入取出与血管粘连的经外周静脉穿刺中心静脉导管经验

摘要

Objective To explore intervention technique for removal of adherent peripherally inserted central catheter (PICC).Methods Interventional experience about removal of adherent PICC in 4 clinical cases between Februa~ 2014 and July 2016 was summarized.Femoral vein was punctured using the technique of Seldinger,whole orbita through Guide wire that was from the distal end of PICC to femoral vein was established,adhesion of PICC was separated gradually using 6F muhipurpose A catheter or long sheath,and then removed.Result PICC was removed successfully in 3 cases,proximal end of PICC was removed in one case,no complications were observed in all of the 4 cases.Conclusion Adherent PICC could be separated and removed effectively though whole orbita using Guide wire.%目的 探讨经导管介入取出与血管粘连滞留体内经外周静脉穿刺中心静脉置管(PICC)导管的经验和技巧.方法 总结2014年2月至2016年7月4例PICC导管因血管粘连滞留体内患儿介入取出经验,穿刺单侧股静脉,应用导丝建立从PICC导管尾端-PICC导管头端侧孔-上腔静脉-右心房-下腔静脉-股静脉全轨道,用导管或长鞘管沿着导丝逐步分离粘连.结果 3例患儿成功取出PICC导管,1例患儿取出PICC导管近心端,4例患儿均无明显不良反应.结论 采用经导管介入建立全轨道方法可有效分离与血管粘连无法顺利拔出滞留体内的PICC导管.
