Objective To evaluate the clinical value of sonographic evaluation and ultrasound-guided fine needle aspiration cytology(US-FNAC) of thyroid nodules. Methods A retrospective review of 169 patients with thyroid nodules was undertaken, including US-FN AC, sonographic evaluation, follow-up, and surgical decision. Results Non-diagnostic FNAC occurred in 24 of 169 (14. 2%) patients, the remaining 145 successfully got the FNAC results, 32 underwent surgery, the accuracy of US-FNAC was 84. 4% ,sensitivity 82. 6%,specificity 88. 9% respectively compared with the pathologic diagnosis. To the accuracy of sonographic evaluation was 89. 0% , sensitivity 94. 7%, specificity 88. 1 %, respectively compared with US-FNAC. The accuracy of sonographic evaluation was 78. 1 %, sensitivity 93. 3% ,specificity 64. 7% respectively compared with the surgical pathologic diagnosis. Conclusions The sonographic evaluation of nodules combined with US-FNAC plays an important role for the management of thyroid nodules.%目的 探讨甲状腺结节超声评估与超声引导下细针穿刺细胞学检查(US-FNAC)的临床应用价值.方法 分析169例甲状腺结节的超声表现、超声引导下甲状腺结节细针穿刺细胞学结果 、随访及手术病理.结果 24例未能获得细胞学诊断(14.2%),145例获得细胞学诊断(85.8%),其中32例手术,与手术病理诊断对照,FNAC的符合率为84.4%,FNAC诊断甲状腺癌的敏感性为82.6% (19/23),特异性为88.90% (8/9).与FNAC对照和手术病理对照,超声符合率分别为89.0% (129/145)和78.1% (25/32),超声诊断甲状腺癌的敏感性分别为94.7% (18/19)和93.3% (14/15),特异性分别为88.1% (111/126)和64.7% (11/17).结论超声评估结合超声引导下细针穿刺细胞学检查对甲状腺结节的正确处理具有重要的临床价值.
