常规及VEGFR2靶向超声造影在结直肠癌肝转移靶向治疗疗效评价中作用研究

摘要

目的 比较常规超声造影与靶向超声造影定量参数在结直肠癌肝转移靶向治疗疗效评价中的作用.方法 建立结直肠癌肝转移裸鼠动物模型并制备VEGFR2靶向造影微泡.30只荷瘤鼠随机分为治疗组及对照组.在治疗前及治疗后7d时进行常规及靶向超声造影检查并得出以下定量参数:达峰时间(TTP)、峰值强度(PI)、曲线上升及下降斜率.同时计算爆破前后PI差值(△PI)以及治疗前△PI与治疗后△PI的差值(△△PI).比较治疗组与对照组间所有定量参数的差别.结果 治疗后7d常规超声造影治疗组仅爆破前PI和爆破后PI显著低于对照组;而在VEGFR2靶向超声造影中,治疗组爆破前PI,爆破后PI、△PI及△△PI均显著低于对照组.结论 靶向超声造影较常规超声造影能更好地判断结直肠癌肝转移靶向治疗疗效.%Objective To explore the clinical value of CEUS perfusion parameters,based on both conventional micro-bubble and targeted micro-bubble,in the treatment evaluation of targeted therapy on colorectal liver metastasis.Methods A model of hepatic metastasis of colon carcinoma in nude mice was established and VEGFR2-targeted microbubble was prepared by biotin-avidin reaction.Thirty tumor bearing nude mice were randomly divided into BV treatment group (n =15) and control group (n=15),and underwent both conventional CEUS and VEGFR2-targeted CEUS,before and 7days after the treatment,respectively.The following parameters were analyzed:TTP (time to peak),PI (peak intensity),slope of rise /fall,pre-blasting PI,post-blasting PI,△PI (post-blasting PI minus pre blasting PI),△△PI (post-treatment △PI minus pre-treatment △PI).Results After 7 days' treatment,with non-targeted CEUS only pre-and post-blasting PI in treatment group were much lower than those in control group.However,with VEGFR2-targeted CEUS,pre-blasting PI,post-blasting PI,△PI,and △△PI in treatment group were all significantly lower than those in control group.Conclusions VEGFR2-targeted CEUS was more effective in treatment assessment in colorectal liver metastasis to BV than conventional CEUS.
