超声引导下莫顿神经瘤注射治疗的临床研究

摘要

目的 对比评估超声引导下复方倍他米松、医用无水乙醇注射治疗莫顿神经瘤的疗效.方法 32例诊断为莫顿神经瘤且自愿行超声引导下注射治疗的患者随机分成两组,A组给予复方倍他米松和利多卡因药物注射,B组给予医用无水乙醇和利多卡因药物注射,于治疗前、治疗后1、3个月对两组患者进行的瘤体大小及疼痛视觉模拟评分(VAS)比较.结果 A、B组病例均成功进行超声引导下药物注射.治疗前A、B组瘤体大小及VAS评分均无统计学差异(均P＞0.05);治疗后1、3个月,A、B组瘤体较治疗前减小、VAS评分较治疗前减低,有统计学差异(均P＜0.05),两组间瘤体大小和VAS评分均无统计学差异(均P＞0.05).结论 超声引导下复方倍他米松、无水乙醇注射治疗莫顿神经瘤都是一种安全、有效的方法,能明显、有效地减轻患者疼痛,提高生活质量,值得临床推广应用.%Objective To evaluate the effect of ultrasound-guided (US-guided) betamethasone,alcohol injection for treatment of Morton's Neuroma.Methods A total of 32 patients who were diagnosed as Morton's Neuroma by ultrasound and clinical and decided to perform US guided Morton's Neuroma injection were romdomly divided into the two groups.Group A were treated with compound betamethasone with lidocaine and group B were treated with alcohol with lidocaine.The size of Morton's Neuroma and visual analogue score (VAS) were compared among prior treatment,1 month and 3 month post-treatment.Results All patients were performed US-guided medicine injection successfully.There were no statistical differences among the sizes of Morton's Neuroma and the VAS scores before treatment (all P＞0.05).At 3 month and 6 month post treatment,the sizes of Morton's Neuroma were decreased and VAS scores were reduced compared with that before treatment,with statistical differences both in group A and B (all P＜0.05),the sizes of Morton's Neuroma and VAS scores were not different between group A and B with no statistical differences (all P＞0.05).Conclusions US-guided Betamethasone and alcohol injection for the treatment of Morton's Neuroma are both a kind of safe and effective method,which can relieve the neuralgia of Morton's Neuroma obviously and effectivt and improve their quality of daily life.
