超声造影对结核性淋巴结与转移性淋巴结的鉴别诊断价值

摘要

目的 探讨超声造影(CEUS)对结核性淋巴结与转移性淋巴结的鉴别诊断价值.方法 回顾性分析80例经病理证实为结核性(32例)和转移性(48例)浅表增大淋巴结的CEUS图像,观察两组淋巴结的CEUS特征.结果 结核组以离心型增强为主,增强早期可见一支门部血管,转移组以向心型增强为主,增强早期可见多支血管,结核组灌注不足区较转移组增强形态规则,边界清楚,增强程度更低,TIC峰值强度和峰值浓度减半时间更低,两组差异均有统计学意义(均P＜0.05);而是否均匀增强及灌注不足区的面积、到达时间和达峰时间均无显著差异(均P＞0.05).结论 CEUS对结核性淋巴结及转移性淋巴结病变的鉴别诊断有一定的价值.%Objective To explore the diagnostic value of contrast-enhanced ultrasound (CEUS) in differential diagnosis of tuberculous and metastatic lymph nodes.Methods The manifestation of CEUS of 80 superficial swollen lymph nodes were analyzed retrospectively.Of 80 lymph nodes,32 tuberculous and 48 metastases.Results Tuberculous group was mainly centrifugal enhancement,one hilum pedicullum been found in early phase,metastatic group was mainly centripetal enhancement,multiple pedicullus been found in early phase,tuberculous group hypoperfusion than metastatic group enhancement shape more rules,enhancement edge more clearly,enhancement degree lower,peak intensity(PI) lower,time from peak to one half(TPH)less,two group had statistically significant(all P＜0.05);however,there was no significance in homogeneity of enhancement,hypoperfusion of area,arrival time (AT),time to peak (TTP)(all P＞0.05).Conclusions CEUS has a certain value in the differential diagnosis tuberculous and metastatic lymph nodes.
