超声心动图在二尖瓣球囊成形术中的应用

摘要

常规应用超声心动图对33例接受二尖瓣球囊成形术治疗的风湿性二尖瓣狭窄病人进行术前病例选择、术中监测与指引房间隔穿刺和术后监测与评估手术效果.房间隔穿刺均成功,扩瓣成功率为94%,术中及时检出心包压塞2例,而使抢救成功.本文认为超声心动图对提高手术成功率,避免和减少并发症均有重要的实用价值.%In 33 patients with rheumatic mitral stenosis undergoing pereutaneons balloon mitral commissurotomy(PBMC),echocardiography was used:1.to guide atrial septal puncture at fossa ovalis,2.to assess effect of PBMC during the operation,to determine the dilating times and regulate the balloon dimension,3.to monitor complications of PBMC.and 4.to assess the indication of PBMC and follow up the result.Two cases with cardiac tamponade was early diagnosed.The success rate of atrial septal puncture and mitarl commissurotomy were 100%and 94%respectively.It is concluded that echocardiography is very useful to increase the success rate and decrease the complication and mortality during PBMC.
