定量彩色室壁运动技术检验左心室负荷rn变化准确性的实验研究

摘要

Objective To evaluate whether quantitative co l or kinesis (QCK) can detect the changes of regional left ventricular systolic and diastolic function under experimental conditions.Methods The echocardiography examinations were performed in 10 d ogs to obtain systole and diastole color kinesis images both at SAX and A4C befo re and after every procedure followed: ①Ligating the inferior vein with stenost ic its diameter 30%;②Ligating the ascending aortic with stenostic its diameter 30%. All the stored digital loops above were reviewed by Echo View computer syst em with QCK software. The regional area changes both of systolic and diastolic ( RACs, RACd),the mean times of systolic and filling would be calculated automatic ally. Results Out of 120 screened color kinesis image frames, 108 ( 90%) with high quality that adequately tracking above 80% endocardial boundary. ①Control condition: The values of RACs [from (38.00±11.29)% for b -lt to (76.33±14.52)% for ant] and RACd[(45.43±19.75)% for b-lt to (77.50±14.72)% for ant] were both different in each se gment . The values got from SAX view were higher. Ts was from(103.40±29.48) ms to (157.44±11.29)ms, and td was from (133.22±21.67)ms t o (170.86±39.51)ms.② Loading condition: After the ligation of inferi or vein, most of RACs(67%) and RACd(75%) were decreased in all segments without changing of global function, ts shorted(83%) and td(75%) prolonged. The changing continued with the ligation of ascending aortic.Conclusions QCK technique can accurately evaluate the LV regiona l systolic and diastolic function under loading, ischemia, and therapy condition s.%目的验证定量彩色室壁运动技术(QCK)软件系统在左心室前后负荷变化等实验状态下,能否准确地显示左心室收缩和舒张功能的变化。方法于10条健康杂种犬,分别在静息状态、结扎下腔静脉使内径减少30% 、结扎升主动脉使内径减少30%等实验条件下采集左心室收缩期和舒张期彩色室壁运动技术(C K)图像。将这些图像回放于配有QCK分析软件的Echo View超声图像处理系统中,该软件系统自动输出左心室各节段收缩期局部面积变化百分率[regional area changes of systol ic ,RACs(%)]或舒张期局部面积变化百分率[regional area changes of diastolic ,RACd(%)]、各节段的平均收缩时间(ts)和平均充盈时间(td)。结果 10条犬获得108幅(90%)满意的左室舒张末期和收缩末期CK图像。①静息对照状态:各节段的RACs[(38.00±11.29)%~(76.33±14.52 )%]和RACd[(45.43±19.75)%~(77.50±14.72)%]数值不同,但比较相近,侧壁基节段最小,前壁中节段最大。各节段收缩期ts[(103.40±29 .48)ms~(157.44±35.73)ms]和舒张期td[(133.22±21.67)m s~(17 0.86±39.51)ms]。②改变负荷状态:结扎下腔静脉后各节段RACs和RACd呈现不同的改变,以减小为主,但总体功能没有明显的变化。收缩各节段的ts有增有减,但以时间缩短占绝大多数;td延长。结扎升主动脉后各节段RACs和RACd减小,但均在正常范围内。该时期各节段的ts缩短, 83%的节段td延长。结论 QCK技术能够准确评价左心室前后负荷改变状态下的左心室局部收缩和舒张功能的变化,为左心室局部收缩和舒张功能的定量分析提供了准确可靠的新途径。
