胎儿左室双出口的产前超声诊断与鉴别诊断

摘要

目的 探讨产前超声诊断左室双出口的临床价值.方法 回顾性检索2005-2016年湖南省5个产前诊断中心胎儿超声检查数据库,分析其超声特点、伴随的心内外畸形和尸体解剖结果进行对照分析.结果 产前超声诊断胎儿左室双出口9例,经尸体解剖证实,8例符合左室双出口,1例为大动脉转位,诊断符合率为89%.左室双出口主要超声特点为主动脉与肺动脉全部或肺动脉大部分起源于形态学左室,两根大血管平行排列,主动脉瓣与肺动脉瓣位于同一水平.结论 产前超声诊断左室双出口具有重要的临床价值,但应与大动脉转位等进行鉴别.%Objective To investigate the clinical value of prenatal diagnosis and differential diagnosis of fetal double outlet left ventricle.Methods The database from 5 fetal ultrasonographic centers were reviewed retrospectively in Hunan from 2005 to 2016 and identified 9 cases of fetal double outlet left ventricle,analyzed the echocardiographic characteristis,associated intracardiac and extracardiac abnormality, and compared with autopsy findings. Results The autopsy findings confirmed the prenatal echocardiographic diagnosis of 8 cases with double outlet left ventricle and one was to be transposition of the great arteries,with diagnose accordance rate being 89%.Echocardiographic findings of fetal double outlet left ventricle was characterized by the origin of the both great arteries arising predominantly or completely from the morphologic left ventricle.Conclusions Prenatal diagnosis of fetal double outlet left ventricle has the important clinical value and it should be differentiated with transposition of the great arteries.
