BACKGROUND: Presbyacusia has no specific therapy at the moment. Whether the introduction of Traditional Chinese Medicine (TCM)Yiyuan fucong(enriching the yuan for hearing recovery) acupuncture therapy can ameliorate and preserve the residual auditions of the patients to effectively inhibit the development of presbyacusia?OBJECTIVE: To observe the effectiveness of yiyuanfucong acupuncture in the patients with presbyacusia.OESIGN: A before-after study.SETTING: Department of TCM of Tianjin Huanhu Hospital.PARTICIPANTS: Eighty ears of 40 cases were all selected from the outpatient department of Tianjin Huanhu Hospital, which were in accordance with the diagnostic criteria for presbyacusia, including 28 male and 12 female patients aged from 60 to 75 years with a course of disease from 6 months to 10 years. There were 26 ears of mild deafness, 36 ears of moderate deafness and 18 ears of serious deafness in our group of patients based on the classification of the deafness, of which 45 ears were complicated with tinnitus.INTERVENTION: All 40 cases received yiyuan fucong acupuncture therapy operated by attending physicians. Point selection: guanyuan, taixi,yifeng, tinghui, zhongzhu, and baihui. Two weeks was set as one therapeutic course. The effectiveness of the treatment was evaluated after 2 courses of continuous therapy.MAIN OUTCOME MEASURES: Quantitative evaluation of the results in audition recovery. Cure: reach the practical standard; marked effectiveness:audition improves 2 grades; effectiveness: audition improves 1 grade; ineffectiveness: audition does not improve or retrograde. Cases with tinnitus:cure: symptom disappears; improvement: symptom reduces; ineffectiveness:no changes or symptom aggravates.RESULTS: Among the 80 ears of 40 cases, cure: 28 ears; marked effectiveness: 7 ears; effectiveness: 32 ears and ineffectiveness: 13 ears. The total ef-fective rate was 84%. In 45 ears complicated with tinnitus, cure: 26 ears (58%), improvement: 14 ears(31% ); and ineffectiveness: 5 ears(11% ). The total effective rate was 89%. No aggravation or depravation was found during the therapy. No audition decrease was found in the 3-month follow-up.CONCLUSION: Yiyuanfucong acupuncture has reliable therapeutic effectiveness in the treatment of presbyacusia.%背景:老年性聋目前尚无特效疗法,采用中医益元复聪法针灸治疗是否能改善和保持患者的残余听力,有效抑制老年性聋的进展.目的:观察益元复聪法针灸治疗老年性聋的疗效.设计:前后对照实验.单位:天津市环湖医院中医科.对象:患者40例80耳均来自本院门诊,符合老年性聋诊断标准.其中男28例,女12例;年龄60～75岁;病程6个月～10年.参照耳聋的分级标准本组病情轻度26耳,中度36耳,重度18耳.伴有耳鸣者45耳.干预:40例患者均由主治医师采用益元复聪法针灸治疗.取穴:关元、太溪、翳风、听会、中渚、百会.2周为1个疗程,连续治疗2个疗程后评判效果.主要观察指标:定量评估听力恢复结果.达到实用标准为治愈;听力提高2级为显效;听力提高1级为有效;听力未提高或退步为无效.伴有耳鸣者,症状消失为治愈,症状减轻为好转,无变化或症状加重为无效.结果:本组40例80耳,治愈28耳,显效7耳,有效32耳,无效13耳,总有效率84%.伴有耳鸣者45耳,治愈26耳,占58%;好转14耳,占31%;无效5耳,占11%;总有效率89%.全部病例在治疗期间无加重或恶化;治疗后随访3个月,无听力下降.结论:益元复聪法针灸治疗老年性聋疗效确切.
