富血小板血浆促进牵拉成骨的矿化过程

     

摘要

背景:富血小板血浆富含多种生长因子,可以促进某些组织的再生。目的:提取兔自体富血小板血浆,将其注射在牵拉间隙内,通过影像学及组织学评价富血小板血浆对兔牵拉成骨矿化过程的作用。方法:32只成年日本大耳白兔,分为4组,每组8只。取兔后肢行胫骨上1/3线锯截骨,制作肢体延长动物模型,分别在建模后5,15,25 d局部注射富血小板血浆500μL 1次,对照组只进行肢体延长,不注射富血小板血浆,术侧肢体完成1 cm胫骨延长。在建模后第35天,处死实验动物,对牵拉新生骨痂进行取材和苏木精-伊红染色,并拍摄兔胫骨正位X射线片。结果与结论:①按照Lane-SandhuX线评价标准评分:建模后15 d注射组、建模后25 d注射组评分明显优于对照组和建模后5 d注射组;②按照Lane-Sandhuz组织学评价标准评分:建模后15 d注射组、建模后25 d注射组组织学评分明显优于对照组和建模后5 d注射组(P<0.05),而建模后25 d注射组的组织学平均评分最高;③结果说明,在兔肢体延长的过程中,提取自体富血小板血浆能够促进骨的矿化成熟,并且在稳定期的中段局部注射,作用最强。%BACKGROUND:The platelet-rich plasma contains various growth factors that could enhance the regeneration of certain tissues. OBJECTIVE:To observe the effect of platelet-rich plasma on the mineralization process during distraction osteogenesis in rabbits. METHODS:Thirty-two adult Japanese white rabbits were randomly divided into four groups (n=8 per group). The upper 1/3 tibial osteotomy at the posterior limb was performed in al rabbits, and the 1 cm limb lengthening models were prepared, fol owed by the local injection of 500μL platelet-rich plasma at 5, 15 and 25 days. The controls received no treatment. At 37 days after modeling, the rabbits were kil ed and newly formed cal us was removed to undergo hematoxylin-eosin staining, and the frontal X-ray film of tibia was obtained. RESULTS AND CONCLUSION:Lane-Sandhu radiographic scoring showed that the scores in the injection groups at 15 and 25 days after modeling were obviously better than those in the control group and the injection at 5 days after modeling. Lane-Sandhu histological scoring revealed that the scores in the injected groups at 15 and 25 days after modeling were significantly higher than those in the control group and group at 5 days after modeling (P<0.05), which highest at 25 days. These results suggest that local injection of platelet-rich plasma can promote bone mineralization during distraction osteogenesis effectively, especial y in the middle period of distraction osteogenesis.
