等速肌力训练对脑卒中偏瘫患者上肢运动功能及日常生活活动能力的影响

         

摘要

Objective To study the effect of isokinetic strength training on upper limb motor function and activities of daily living (ADL) in stroke patients with hemiplegia. Methods From January to December, 2016, 56 stroke patients with hemiplegia were randomly di-vided into control group and observation group equally. Both groups received routine rehabilitation training, and the observation group ac-cepted hemiplegic limb flexor and extensor muscle isokinetic strength training in addition, for four weeks. They were assessed with Fugl-Meyer Assessment-upper limb (FMA-UL), modified Ashworth Scale (MAS), modified Barthel Index (MBI) and biomechanical index-es of elbow flexion and extension with Biodex System-4 before and after treatment. Results The scores of FMA and MBI improved after treatment (t>7.083, P<0.001) in both groups, and improved more in the observation group than in the control group (t>2.369, P<0.05). The peak torque, total work and average power at 60°/s and 120°/s angular velocity of both flexor and extensor improved in both groups (t>2.149, P<0.05), and those of the extensor improved more in the observation group than in the control group (t>2.027, P<0.05), except the peak torque at 60°/s angular velocity. Conclusion Isokinetic strength training can improve the motor function of upper limbs and activities of daily living in stroke patients with hemiplegia.%目的 研究等速肌力训练对脑卒中偏瘫患者上肢运动功能及日常生活活动能力的影响.方法 2016年1月至12月,脑卒中偏瘫患者56例随机等分为对照组和观察组,两组均予常规康复训练,观察组加患侧肘关节屈伸肌群等速肌力训练,共4周.治疗前后采用Fugl-Meyer评定量表(FMA)上肢部分、改良Ashworth量表(MAS)、改良Barthel指数(MBI)、Biodex System-4等速测试训练系统进行评定.结果 治疗后,两组患者上肢FMA评分、MBI评分均明显提高(t>7.083,P<0.001),观察组优于对照组(t>2.369,P<0.05).两组在60°/s、120°/s角速度时,肘屈肌、伸肌峰力矩、总功和平均功率均提高(t>2.149,P<0.05),观察组120°/s角速度时伸肌峰力矩,60°/s、120°/s角速度时伸肌总功和平均功率优于对照组(t>2.027,P<0.05).结论 等速肌力训练可以改善脑卒中偏瘫患者上肢运动功能,提高患者日常生活活动能力.
