抑郁症患者言语流畅性任务下近红外光谱脑激活特征的初步研究

摘要

目的 探讨抑郁症患者言语流畅性任务下近红外光光谱(near-infrared spectroscopy,NIRS)的脑激活特征差异.方法 选择语义相关的言语流畅性任务(verbal fluency task,VFT)作为刺激任务,对14例抑郁症患者(患者组)和13名性别、年龄、受教育年限相匹配的健康对照者(对照组)进行测试,利用FOIRE-3000进行脱氧血红蛋白(deoxygenate hemoglobin,Deoxy-Hb)和氧合血红蛋白(oxygenate hemoglobin,0xy-Hb)相对浓度的数据采集,应用SPM软件包对Deoxy-Hb和Oxy-Hb的相对浓度变化值进行数据处理.结果 在任务执行结果无明显差异的前提下,患者组Oxy-Hb和Deoxy-Hb的平均曲线较对照组低平,差异主要位于前额叶背外侧区,如38通道和41通道(相当于brodmann分区的46区,前额叶背外侧区)的Deoxy-Hb(t=3.29、3.47)和Oxy-Hb(t =3.18、4.03)相对浓度变化均P<0.10.结论 在选择合适的认知任务条件下,抑郁症患者双侧前额叶背外侧区激活程度下降.%Objective To investigate the difference of brain activation patterns between depression patients and normal persons by near-infrared spectroscopy imaging.Methods Totally 14 depressed patients and 13 sex,age,and education matched healthy controls were required to perform category verbal fluency task (VFT).The relative concentration of deoxygenate hemoglobin and oxygenate hemoglobin was collected by a 45-channel instrument,and the data was processed with the software of Statistical Parametric Mapping (SPM).Results The task performances were not significantly different between the two groups,the average curve of oxygenate hemoglobin and deoxygenate hemoglobin were lower in depressed patients with significant difference in dorsolateral prefrontal cortex.For instance,the oxygenate change in Channel 38 and Channel 41 was Deoxy-Hb(t =3.29,3.47),Oxy-Hb (t =3.18,4.03),P < 0.10.Conclusion The activation in bilateral dorsolateral prefrontal cortex may be lower in depression patients compared to control group.
